贵州省地州市县级公立医院重症医学科规范建设的现场调查研究

摘　　要：目的调查贵州省各地州市县重症医学科的规范建设化情况,以促进贵州省重症医学同质化发展。方法采用现场调查、实地采集数据核准的研究方法,首先将贵州省9个地州市88个县市区的公立医院分为东南、东北、西北、西南、贵阳市5条路线进行分批调查研究,设计重症医学科规范建设的调查表格,电子表格提前2~3 d通过邮箱和微信发给重症医学科主任或其指定人员完成填写,现场逐项核对数据真实性,核对完成收取表格离开医院。结果2021年4至7月完成贵州省9个地州市88个县市区建有重症医学科的146家公立医院(省级医院除外)的调查研究数据采集,其中包括24家三级甲等医院,122家二级及以上医院(包括三级乙等8家、三级综合11家、二级甲等97家、二级乙等3家、二级综合3家)。146家公立医院总编制床位80983张,总开放床位104017张。重症医学科建有实际床位2035张,重症医学科实际床位占医院编制总床位的2.51%。1999至2010年建科的有18家(占12.33%),2011至2021年建科的有128家(占87.67%)。学科总面积113355.48 m^(2),平均每床面积55.70 m^(2)。病床间距:1.5~2.0 m的有97家医院(占66.44%),2.1~>2.5 m的有49家(占33.56%)。负压病房及数量:43家医院各有1间负压病房(占29.45%),103家医院无负压病房(占70.55%)。单间病房数量:140家医院有288间单间病房(占95.89%),6家医院无单间病房(占4.11%)。中心供氧:138家医院有中心供氧(占94.52%),8家医院无中心供氧(占5.48%)。自然通风:129家医院有自然通风设施(占88.36%),17家医院无占自然通风设施(11.64%)。专科重症监护病房(ICU)建设:66家医院有专科ICU(占45.21%),80家医院无专科ICU(占54.79%)。146家公立医院有医师和护士共3712名,其中医师总人数1041名,医师与床位比为0.51∶1;护士总人数2675名,护士与床位比为1.31∶1。结论贵州省9个地州市88个县市区均建有重症医学科,学科规范化�Objective To investigate the standardized construction of critical care departments in different cities and counties of Guizhou province to promote the homogenization development of critical care medicine in Guizhou Province.Methods Using research methods such as field investigation and data collection,the public hospitals of 88 counties and urban areas in 9 prefectures and cities of Guizhou province were divided into five routes:southeast,northeast,northwest,southwest,and Guiyang.To design the survey form for the standardized construction of ICU,the e-form was sent to the director of ICU or his/her designated personnel by email or wechat 2-3 days in advance.Check the authenticity of data item by item on site,and leave the hospital after checking the receipt form.Results From April to July 2021,the survey and research data collection was completed for 146 public hospitals(excluding provincial hospitals)with intensive care departments in 88 counties and cities of 9 dizhou cities in Guizhou Province,including 24 Grade-ⅢLevel A hospitals.122 Grade-Ⅱand above hospitals(including 8 Grade-ⅢLevel B hospitals,11 Grade-Ⅲcomprehensive hospitals,97 Level-ⅡA hospitals,3 Level-ⅡB hospitals,and 3 Level-Ⅱcomprehensive hospitals).146 public hospitals have a total of 80983 beds and 104017 open beds.The department of Critical Care has 2035 beds.The ratio of actual beds in ICU to total beds in hospital was 2.51%.From 1999 to 2010,18(12.33%)established departments,and from 2011 to 2021,128(87.67%)established departments.The total area of the discipline is 113355.48 m2,with an average bed area of 55.70 m2.There were 97 hospitals with 1.5-2.0 m bed spacing,accounting for 66.44%,and 49 hospitals with 2.1->2.5 m spacing,accounting for 33.56%.The number of negative pressure wards:1 in each of 43 hospitals,accounting for 29.45%;103 hospitals did not have,accounting for 70.55%.The number of single rooms:288 in 140 hospitals,accounting for 95.89%;6 hospitals did not have,accounting for 4.11%.Central oxygen supply:138 hospitals
