基于立体定向脑电图技术的数字化脑电图机检测信号与颞叶癫痫智商指标改变的关系研究

摘　　要：目的:研究基于立体定向脑电图(SEEG)技术的数字化脑电图机可靠性脑电分析信号值与颞叶癫痫(TLE)智商指标改变的相关性。方法:选取医院收治的72例TLE患者,根据韦氏智力量表评分将其分为智力减退组(37例)和不伴智力减退组(35例),详细记录两组患者的病史、体征、视频脑电图(VEEG)数据,采用数字化脑电图机进行SEEG检测,量化处理患者脑电数据。对患者TLE发作间期SEEG脑电信号与韦氏智力量表评分结果进行Pearson相关分析。结果:Pearson相关分析结果显示,韦氏智力量表中言语智商(vIQ)评分与颞中回前部SEEG信号数据中的波幅绝对平均值、波幅标准方差、全频段总功率、Delta功率及theta功率呈显著负相关(r=-0.410,r=-0.402,r=-0.217,r=-0.320,r=-0.191;P<0.05);操作智商(pIQ)评分与颞中回前部波幅绝对平均值、波幅标准方差、全频段总功率、alpha功率和Delta功率呈显著负相关(r=-0.277,r=-0.412,r=-0.206,r=-0.319,r=-0.181;P<0.05);智力减退组波幅绝对平均值和波幅标准方差明显增高,与不伴智力减退组比较差异均有统计学意义(t=2.178,t=4.246;P<0.05)。结论:SEEG脑电变异值与TLE患者智商指标改变情况具有相关性,基于SEEG技术的数字化脑电图机可靠性脑电信号分析在TLE智商损害评估中具有重要的检测价值。Objective:To research the correlation between the signal value of reliable electroencephalograph(EEG) analysis of digital EEG based on stereoelectroencephalography(SEEG) and the change of intelligence quotient(IQ) parameters of temporal lobe epilepsy(TLE).Methods:72 TLE patients admitted to hospital were included in the retrospective analysis.And they were divided into hypophrenia group(37 cases) and non-hypophrenia group(35 cases) according to the scores of Wechsler intelligence scale,and the medical history,physical signs,video electroencephalography(VEEG) data of two groups were recorded in detail.And digital EEG was adopted to perform SEEG detection,and to quantitatively process EEG data of patients.Pearson correlation analysis was adopted to analyze the relationship between SEEG signals during TLE interictal phase and the results of the scores of Wechsler intelligence scale in patients.Results:The results of Pearson correlation analysis showed that verbal IQ(vIQ) score of Wechsler intelligence scale was significantly negative correlated with absolute mean of amplitude,standard variance of amplitude,total power of whole band,Delta power and Theta power of SEEG signal data of anterior middle temporal gyrus(r=-0.410,r=-0.402,r=-0.217,r=-0.320,r=-0.191,P<0.05).The performance IQ(pIQ) scores were significantly negative correlated with absolute mean amplitude,standard variance of amplitude,total power of whole band,Alpha power and Delta power of anterior middle temporal gyrus(r=-0.277,r=-0.412,r=-0.206,r=-0.319,r=-0.181,P<0.05).The absolute mean of amplitude and the standard variance of amplitude were significantly increased in hypophrenia group,and the differences of them between two groups were significant(t=2.178,t=4.246,P<0.05).Conclusion:SEEG EEG variation value is correlated with the changes of IQ indices of TLE patients.The digital EEG reliable signal analysis based on SEEG technique has important detection value in the assessment of IQ impairment of TLE patients.
