摘　　要：目的比较经口腔前庭入路与全乳晕入路腔镜单侧甲状腺癌手术的临床效果及术中出血量。方法选取100例甲状腺癌手术患者作为研究对象,采用电脑随机方式将其分为观察组(50例,经口腔前庭入路腔镜单侧甲状腺癌手术)和对照组(50例,全乳晕入路腔镜单侧甲状腺癌手术)。比较两组的治疗效果。结果观察组的术中失血量少于对照组,手术时长、住院时间短于对照组,中央区淋巴结清扫数量多于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。治疗后,观察组的红细胞沉降率、C-反应蛋白水平、VAS评分均低于对照组,白细胞计数高于对照组(P<0.05)。观察组的治疗总有效率、术后美观满意度高于对照组(P<0.05)。结论经口腔前庭入路腔镜单侧甲状腺癌手术的临床效果优于全乳晕入路,且术中出血量更少。Objective To compare the clinical effect and intraoperative blood loss between transoral vestibular approach and full areola approach in endoscopic unilateral thyroid cancer surgery.Methods A total of 100 patients with thyroid cancer surgery were selected as the research objects and randomly divided into observation group(50 cases,transoral vestibular approach for endoscopic unilateral thyroid cancer surgery)and control group(50 cases,full areola approach for endoscopic unilateral thyroid cancer surgery)by computerized randomization.The therapeutic effects of the two groups were compared.Results The intraoperative blood loss in the observation group was less than that in the control group,the operation time and hospital stay were shorter than those in the control group,and the number of central lymph node dissection was more than that in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).After treatment,the erythrocyte sedimentation rate,C-reactive protein level and VAS score in the observation group were lower than those in the control group,and the leukocyte count was higher than that in the control group(P<0.05).The total effective rate of treatment and postoperative aesthetic satisfaction in the observation group were higher than those in the control group(P<0.05).Conclusion The clinical effect of transoral vestibular approach for endoscopic unilateral thyroid cancer surgery is better than that full areola approach,and the intraoperative blood loss is less.
