血清PCT、CRP和IL-6联合检测对肝硬化合并腹水感染的诊断价值

摘　　要：目的探讨血清降钙素原(PCT)、C-反应蛋白(CRP)和白细胞介素-6(IL-6)联合检测对肝硬化合并腹水感染的诊断价值。方法选择西电集团医院2018年1月至2019年6月收治的50例肝硬化合并腹水感染患者为试验组,50例单纯肝硬化患者为对照1组,50例同期进行健康体检者为对照2组。所有研究对象均接受血清PCT、CRP和IL-6水平检测。比较三组研究对象的血清PCT、CRP以及IL-6水平;分析试验组的CRP、IL-6、PCT单项及联合检测的诊断价值。结果试验组的血清PCT、CRP及IL-6水平均显著高于对照1组及对照2组,差异具有统计学意义(P<0.05);对照1组的血清PCT水平与对照2组比较,差异无统计学意义(P>0.05);对照1组的血清CRP、IL-6水平均显著高于对照2组,差异具有统计学意义(P<0.05)。CRP+IL-6+PCT检测的灵敏度、特异度、准确度、阳性预测值及阴性预测值均显著高于CRP、IL-6、PCT、CRP+IL-6、CRP+PCT、IL-6+PCT检测,差异具有统计学意义(P<0.05)。结论血清PCT、CRP和IL-6联合检测诊断肝硬化合并腹水感染患者的临床价值显著,值得推广应用。Objective To explore the diagnostic value of combined detection of serum procalcitonin(PCT),C-reactive protein(CRP)and interleukin-6(IL-6)in liver cirrhosis complicated with ascites infection.Methods A total of 50 patients with liver cirrhosis complicated with ascites infection admitted in Xi'an XD group hospital from January 2018 to June 2019 were selected as experimental group,50 patients with simple liver cirrhosis as control group 1,and 50 people undergoing health screening in the same period as control group 2.All research objects were tested for the levels of serum PCT,CRP and IL-6.The levels of serum PCT,CRP and IL-6 were compared among the three groups;the diagnostic value of single and combined detection of CRP,IL-6 and PCT in the experimental group were analyzed.Results The levels of serum PCT,CRP and IL-6 in the experimental group were significantly higher than those in the control group 1 and the control group 2,and the differences were statistically significant(P<0.05);there was no significant difference in serum PCT level between the control group 1 and the control group 2(P>0.05);the levels of serum CRP and IL-6 in the control group 1 were significantly higher than those in the control group 2,and the differences were statistically significant(P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of CRP+IL-6+PCT detection were significantly higher than those of CRP,IL-6,PCT,CRP+IL-6,CRP+PCT and IL-6+PCT detection,and the differences were statistically significant(P<0.05).Conclusion The combined detection of serum PCT,CRP and IL-6 in the diagnosis of patients with liver cirrhosis complicated with ascites infection has significant clinical value,which is worthy of promotion and application.
