NLR、MLR、PLR水平对胃癌术后短期复发及转移的预测价值

摘　　要：目的探讨中性粒细胞/淋巴细胞比率(NLR)、淋巴结转移率(MLR)、血小板/淋巴细胞比率(PLR)在胃癌术后短期复发转移中的预测价值。方法选取手术治疗的胃癌患者130例,收集患者临床随访资料,检测计算NLR、MLR、PLR水平,分析其对胃癌术后复发及转移的预测价值。结果130例患者中,术后3年复发转移患者45例,复发转移率为34.62%。复发转移患者TNM分期Ⅲ期比例、中低分化比例、NLR、MLR和PLR分别为73.33%、68.89%、(2.84±0.95)、(0.47±0.12)和(165.11±63.32),明显高于无复发转移患者(P<0.05)。NLR、MLR和PLR预测胃癌术后复发转移的ROC曲线下面积为0.709、0.833和0.698,NLR、MLR和PLR截断值分别为2.45、0.31和106.36时,灵敏性分别为57.80%、87.80%和66.80%,特异性分别为78.18%、65.50%和65.50%。结论NLR、MLR、PLR在胃癌术后复发转移预测中有一定应用价值,值得进一步研究。Objective To explore the predictive value of neutrophil/lymphocyte ratio(NLR),lymph node metastasis rate(MLR),platelet/lymphocyte ratio(PLR)in short-term recurrence and metastasis of gastric cancer after operation.Methods 130 patients with gastric cancer who underwent surgical treatment were selected and collected clinical follow-up data.Results Among 130 patients,45 had recurrence and metastasis at 3 years after operation,the recurrence and metastasis rate was 34.62%;The proportions of TNM stageⅢ,moderate to low differentiation,NLR,MLR and PLR in patients with recurrence and metastasis were 73.33%,68.89%,(2.84±0.95),(0.47±0.12)and(165.11±63.32),which were significantly higher than those in patients without recurrence and metastasis(P<0.05);The area under ROC curve of NLR,MLR and PLR for predicting recurrence and metastasis of gastric cancer after operation were 0.709,0.833 and 0.698,P<0.05,when the cut-off values of NLR,MLR and PLR were 2.45,0.31 and 106.36,the sensitivity were 57.80%,87.80%and 66.80%,the specificity were 78.18%,65.50%and 65.50%.Conclusion NLR,MLR and PLR have certain application value in predicting recurrence and metastasis of gastric cancer after operation,which is worth further study.
