摘 要:目的探究多模态超声联合血管内皮生长因子(VEGF)、原癌基因(CerbB-2)在非哺乳期乳腺炎(NLM)与乳腺癌鉴别诊断中的临床价值。方法回顾性分析63例乳腺癌、57例NLM患者的多模态超声检查资料及血清VEGF、CerbB-2水平,以病理诊断为金标准,分析多模态超声联合血清VEGF、CerbB-2水平对非哺乳期乳腺炎与乳腺癌的鉴别诊断价值。结果常规超声鉴别诊断NLM和乳腺癌的灵敏度、特异度、准确度为71.43%、56.14%、64.17%,UE(超声弹性成像)诊断的灵敏度、特异度、准确度为84.13%、68.42%、76.67%,CEUS诊断的灵敏度、特异度、准确度为79.37%、75.44%、77.50%,UE、CEUS(超声造影)诊断效能均高于常规超声(P<0.05);VEGF鉴别诊断NLM和乳腺癌的灵敏度、特异度、准确度为84.13%、54.39%、70.00%,CerbB-2诊断灵敏度、特异度、准确度为85.71%、50.88%、69.17%,多模态超声联合VEGF、CerbB-2诊断灵敏度、特异度、准确度为93.65%、89.47%、91.67%,高于单一检测方法(P<0.05)。结论NLM无特异性临床表现,仅靠二维超声容易误诊,采用多模态超声联合VEGF、CerbB-2能有效对NLM和乳腺癌进行鉴别诊断。Objective To explore the clinical value of multimodal ultrasound combined with vascular endothelial growth factor(VEGF)and proto-oncogene(CerbB-2)in the differential diagnosis of non-lactating mastitis(NLM)and breast cancer.Methods The multimodal ultrasound data and serum VEGF and CerbB-2 levels of 63 breast cancer and 57 NLM patients were selected.The value of multimodal ultrasound combined with serum VEGF and CerbB-2 levels in the differential diagnosis of non-lactating mastitis and breast cancer was analyzed,pathological diagnosis as the gold standard.Results The sensitivity,specificity,and accuracy of conventional ultrasound in the differential diagnosis of NLM and breast cancer were 71.43%,56.14%,and 64.17%;the sensitivity,specificity,and accuracy of UE diagnosis were 84.13%,68.42%,and 76.67%;the sensitivity,specificity,and accuracy of CEUS were 79.37%,75.44%,and 77.50%.The diagnostic efficiency of UE and CEUS were significantly higher than those of conventional ultrasound(P<0.05).The sensitivity,specificity and accuracy of VEGF in the differential diagnosis of NLM and breast cancer were 84.13%,54.39%,70.00%;diagnostic sensitivity,specificity and accuracy of CerbB-2 were 85.71%,50.88%,69.17%;the diagnostic sensitivity,specificity and accuracy of multimodal ultrasound combined with VEGF and CerbB-2 were 93.65%,89.47%,91.67%,which were higher than the single detection method(P<0.05).Conclusion NLM has no specific clinical manifestations,and it is easy to be misdiagnosed by two-dimensional ultrasound alone.Multimodal ultrasound combined with VEGF and CerbB-2 can effectively differentiate between NLM and breast cancer.
