摘 要:目的探讨鼻咽癌患者外周血趋化因子12(CXCL12)、CXCL14表达与同步放化疗效果的相关性。方法回顾性分析同步放化疗的80例鼻咽癌患者临床资料,根据治疗结果分为控制与未控制两组,比较两组治疗前后外周血CXCL12、CXCL14表达情况,分析外周血CXCL12、CXCL14表达对鼻咽癌患者同步放化疗效果的影响,探讨外周血CXCL12、CXCL14表达与同步放化疗效果的关系。结果80例鼻咽癌患者经同步放化疗控制率为71.25%;未控制组病程长于控制组,TNM分期级别高于控制组;治疗3个周期,两组外周血CXCL12表达均较治疗前降低,CXCL14表达升高,且未控制组各时点外周血CXCL12高于控制组,CXCL14低于控制组,差异有统计学意义(P<0.05);经回归分析检验结果显示,病程、TNM分期、外周血CXCL12高表达、CXCL14低表达是鼻咽癌患者同步放化疗控制效果不佳的影响因素(OR>1,P<0.05);绘制ROC曲线,外周血CXCL12、CXCL14表达分别用于鼻咽癌患者同步放化疗效果预测的AUC分别为:0.804、0.872,均有一定预测价值。结论CXCL12过表达、CXCL14低表达可能会对鼻咽癌患者同步放化疗实施效果产生影响,增加病情控制不佳风险,早期监测鼻咽癌患者外周血CXCL12、CXCL14水平可预测鼻咽癌治疗风险,可能对指导早期治疗方案拟定、确保治疗效果有积极意义。Objective To explore the correlation between the expression of chemokine 12(CXCL12)and CXCL14 in patients with nasopharyngeal carcinoma and the effect of concurrent radiochemotherapy.Methods The clinical data of 80 patients with nasopharyngeal carcinoma who underwent concurrent radiotherapy and chemotherapy were retrospectively analyzed,according to the therapeutic results,they were divided into a controlled group and an uncontrolled group.The expression of CXCL12 and CXCL14 in peripheral blood before and after treatment between the 2 groups was compared,the effect of peripheral blood CXCL12 and CXCL14 expression on the efficacy of simultaneous radiochemotherapy in patients with nasopharyngeal carcinoma was analyzed,the relationship between the expression of CXCL12 and CXCL14 in peripheral blood and the effect of simultaneous radiotherapy and chemotherapy was explored.Results The control rate of 80 patients with nasopharyngeal carcinoma after concurrent radiotherapy and chemotherapy was 71.25%;the duration of the uncontrolled group was longer than that of the control group,and the TNM staging level was higher than that of the control group;after 3 cycles of treatment,the expression of CXCL12 in the peripheral blood of the 2 groups was lower than that before the treatment,and the expression of CXCL14 was increased,and the CXCL12 of the peripheral blood was higher than that of the control group and CXCL14 was lower than that of the control group at each time point,the differences were statistically significant(P<0.05);the results of regression analysis showed that the course of disease,TNM stage,high expression of CXCL12 in peripheral blood,and low expression of CXCL14 were the influencing factors of poor control of concurrent radiotherapy and chemotherapy in patients with nasopharyngeal carcinoma(OR>1,P<0.05);the ROC curve was drawn,the expressions of CXCL12 and CXCL14 in peripheral blood were used to predict the AUC of nasopharyngeal carcinoma patients with concurrent radiochemotherapy effects was:0.804 and 0.872
