双能CT影像组学指标对进展期胃癌脉管生成及临床预后的诊断评估价值分析

摘　　要：目的探讨双能CT影像组学指标在进展期胃癌脉管生成及临床预后评估中的价值。方法以80例进展期胃癌患者为研究对象,行双能CT检查,并检测微血管密度(MVD)。比较不同MVD值患者的碘浓度(IC)、标准化碘浓度(nIC)值及能谱曲线的斜率,分析其与MVD值的相关性,并采用ROC曲线分析双能CT影像组学指标对进展期胃癌脉管浸润及预后的诊断评估价值。结果高MVD组IC、nIC值及斜率大于低MVD组(P<0.05);IC、nIC值与MVD值呈正相关(P<0.05);脉管浸润组IC、nIC值及斜率大于非浸润组(P<0.05)。IC、nIC值及斜率诊断进展期胃癌脉管浸润的AUC大于0.7;死亡组IC、nIC值及斜率大于生存组(P<0.05)。IC、nIC值预测患者预后的AUC大于0.7。结论进展期胃癌患者IC、nIC值与脉管生成相关,且对进展期胃癌脉管浸润及预后具有诊断评估价值。Objective To explore the value of dual-energy CT imaging omics in angiogenesis and clinical prognosis evaluation of advanced gastric cancer.Methods 80 patients with advanced gastric cancer were enrolled as the research objects.All underwent dual-energy CT examination and were tested for microvessel density(MVD).The iodine concentration(IC),normalized iodine concentration(nIC)and slopes of energy spectrum curves in patients with different MVD values were compared.And their correlation with MVD value was analyzed.The value of dual-energy CT imaging omics indexes in vascular invasion and prognosis evaluation of advanced gastric cancer was analyzed by ROC curves.Results IC,nIC and slopes in high MVD group were greater than those in low MVD group(P<0.05).IC and nIC were positively correlated with MVD(P<0.05).IC,nIC and slopes in vascular invasion group were greater than those in non-invasion group(P<0.05).AUC values of IC,nIC and slopes in the diagnosis of vascular invasion were greater than 0.7.IC,nIC and slopes in death group were greater than those in survival group(P<0.05).AUC values of IC and nIC for predicting prognosis were greater than 0.7.Conclusion IC and nIC are related to vasculogenesis in patients with advanced gastric cancer,which are of diagnosis and evaluation value for vascular invasion and prognosis of advanced gastric cancer.
