皮肤镜与Mohs手术治疗皮肤基底细胞癌患者的疗效观察

摘　　要：目的分析皮肤镜与显微描记(Mohs)手术治疗皮肤基底细胞癌的疗效。方法选取皮肤基底细胞癌患者78例,按随机数字表法分为试验组(n=39)和对照组(n=39),对照组予以Mohs手术,试验组予以皮肤镜与Mohs手术。对比两组送检次数、手术时间、肿瘤相关指标、扩切2 cm时切缘性质、复发率。结果试验组送检次数较对照组少,手术时间较对照组短(P<0.05);试验组肿瘤被切除后皮肤缺损面积较对照组小,首次切缘距离肿瘤边缘最大距离、首次切缘距离肿瘤边缘最小距离较对照组短(P<0.05);试验组扩切2 cm时切缘阳性10.26%(4/39),较对照组[28.21%(11/39)]低,阴性[89.74%(35/39)]较对照组[71.79%(28/39)]高(P<0.05);试验组复发率[2.63%(1/38)]较对照组[22.22%(8/36)]低(P<0.05)。结论采用皮肤镜与Mohs手术治疗皮肤基底细胞癌患者,可减少送检次数,缩短手术时间,缩短首次切缘距离肿瘤边缘最大距离、首次切缘距离肿瘤边缘最小距离,缩小皮肤缺损面积,并可提高扩切切缘阴性率,降低术后复发率。临床可据此实施术后措施,可以更好地做好患者皮损护理,以利于患者皮损恢复,并做好患者心理及生活指导,以提高患者术后生存质量,改善预后。Objective To analyze the curative effect of dermoscopy and microscopic Mohs on basal cell carcinoma of skin.Methods 78 patients with basal cell carcinoma of skin,were divided into the experimental group(n=39)and the control group(n=39),The control group received Mohs surgery,the experimental group received Dermoscopy and Mohs surgery.Examination times,operation time,tumor-related indexes and margin properties of 2 cm were compared.Results The experimental group had fewer examination times than the control group,operation time was shorter than that of control group(P<0.05);The area of skin defect after tumor resection in the experimental group was smaller than that in the control group(P<0.05),the maximum and the miminal distance between the first cut edge and the tumor margin were shorter than the control group(P<0.05);positive rate of margin 2 cm wider in the experimental group,10.26%(4/39)was lower than the control group 28.21%(11/39),negative rate in the experimental group,89.74%(35/39)was higher than that of the control group 71.79%(28/39)(P<0.05).The recurrence rate in the experimental group[2.63%(1/38)],was lower than that of the control group[22.22%(8/36)],(P<0.05).Conclusion The treatment of skin basal cell carcinoma by dermoscopy and Mohs surgery can reduce the number of inspections,shortening the operation time,shorten the maximum and the miminal distance between the first cut edge and the tumor margin,reduce the area of skin defects,and improve the negative rate of extended cutting edge,reduce postoperative recurrence rate.
