环状软骨上喉部分切除术治疗中晚期喉癌的临床效果

摘　　要：目的探究环状软骨上喉部分切除术在中晚期喉癌治疗中的效果。方法选择82例中晚期喉癌患者,按随机数字表法分为2组,各41例。对照组行全喉切除术,研究组行环状软骨上喉部分切除术。比较2组患者术后各指标恢复情况、并发症发生率、生存率及复发率。结果研究组术后引流量为(83.70±10.86)ml,少于对照组的(105.59±23.63)ml;喉功能恢复时间为(14.20±1.31)d、鼻饲时间为(15.79±3.22)d、试吃开始时间为(16.80±3.57)d、住院时间为(20.43±3.96)d,均短于对照组的(19.85±1.54)d、(18.95±3.51)d、(20.19±3.62)d、(25.88±4.12)d,差异有统计学意义(P<0.05)。研究组术后并发症发生率为4.88%,低于对照组的24.39%,差异有统计学意义(P<0.05)。研究组生存率为73.17%、复发率为39.02%,对照组生存率为68.29%、复发率为41.46%,2组比较无统计学差异(P>0.05)。结论环状软骨上喉部分切除术在中晚期喉癌患者治疗中疗效确切,患者术后恢复较快,并发症少,生存率高,安全可靠。Objective To explore the effect of partial laryngectomy on cricoid cartilage in the treatment of advanced laryngeal cancer.Methods 82 patients with advanced laryngeal cancer were randomly divided into 2 groups,41 cases in each group.Total laryngectomy was performed in the control group and partial laryngectomy in the study group.The postoperative recovery,complication rate,survival rate and recurrence rate were compared between the 2 groups.Results The postoperative drainage volume in the study group was(83.70±10.86)ml,which was less than(105.59±23.63)ml in the control group;The recovery time of laryngeal function was(14.20±1.31)d,nasal feeding time was(15.79±3.22)d,trial eating start time was(16.80±3.57)D and hospital stay was(20.43±3.96)d,which were shorter than those of the control group(19.85±1.54)d,(18.95±3.51)d,(20.19±3.62)D and(25.88±4.12)d(P<0.05).The incidence of postoperative complications in the study group was 4.88%,lower than 24.39%in the control group(P<0.05).The survival rate was 73.17%and the recurrence rate was 39.02%in the study group and 68.29%and 41.46%in the control group.There was no significant difference between the 2 groups(P>0.05).Conclusion Partial laryngectomy with cricoid cartilage is effective in the treatment of patients with advanced laryngeal cancer,with rapid postoperative recovery,less complications,high survival rate,safety and reliability.
