摘　　要：目的:探讨双切口囊外白内障摘除术(ECCE)对低角膜内皮细胞数硬核患者术后的影响。方法:选取2018年6月~2020年6月在本院确诊的80例(80眼)低角膜内皮细胞数硬核白内障患者,简单随机分组为观察组和对照组,每组各40例(40眼)。对照组采用单切口ECCE,观察组采用双切口ECCE,记录手术时间、术中出血量及切口愈合时间等手术相关指标,分别于手术前后评估患者的角膜内皮密度、视力、散光;记录角膜内皮细胞丢失率、六边形细胞比例及并发症的发生率。结果:两组患者的手术时间、术中出血量、切口愈合时间、视力及散光无显著差异(均P>0.05);在术后1个月,观察组角膜内皮细胞密度显著高于对照组,丢失率及六边形细胞比例显著低于对照组(均P<0.05);观察组并发症的发生率为2.50%,显著低于对照组的20.00%(P<0.05)。结论:双切口ECCE可降低低角膜内皮细胞数硬核白内障患者术后角膜内皮细胞的损伤,降低角膜内皮功能失代偿的发生率。Objective:To explore the effect of double-incision extracapsular cataract extraction(ECCE)on postoperative conditions in patients with hard-nucleus cataract of low corneal endothelial cell density.Methods:A total of 80 patients(80 eyes)of hard-nucleus cataract with low corneal endothelial cell density confirmed in our hospital between June 2018 and June 2020 were enrolled.According to simple random grouping method,they were divided into the observation group and the control group,with 40 cases(40 eyes)in each group.The patients in the control group were treated with single-incision ECCE,while the observation group with double-incision ECCE.The surgical related indexes,including operation time,intraoperative blood loss,incision healing time were recorded.Before and after surgery,corneal endothelial density,visual acuity and astigmatism were evaluated.The loss rate of corneal endothelial cells,proportion of hexagonal cells and incidence of complications were recorded.Results:There was no significant difference in operation time,intraoperative blood loss,incision healing time,visual acuity or astigmatism between the two groups(all P>0.05).At 1 month after surgery,density of corneal endothelial cells in the observation group was significantly higher than that in the control group,while loss rate of corneal endothelial cells and proportion of hexagonal cells were significantly lower than those in the control group(all P<0.05).The incidence of complications in the observation group was significantly decreased,when compared with the control group(2.50%vs 20.00%,P<0.05).Conclusion:Double-incision ECCE can reduce the injury of corneal endothelial cells and the incidence of corneal endothelial decompensation in patients with hard-nucleus cataract of low corneal endothelial cell density.
