摘 要:目的探讨SLeX和Maspin表达与乳腺浸润性导管癌(BIDC)临床病理指标及预后的关系。方法80例BIDC手术切除标本及30例距癌灶3 cm的癌旁乳腺组织,应用EliVision^(TM) plus免疫组织化学二步法检测SLeX和Maspin在癌组织及癌旁乳腺组织中的表达,分析SLeX和Maspin与病人年龄、TNM分期、分化等级、淋巴结宏转移、术后5年内复发转移等临床病理指标的相关性。结果癌组织SLeX和Maspin阳性率分别为86.3%(69/80)和47.5%(38/80),癌旁乳腺组织分别为20.0%(6/30)和93.3%(28/30),差异有统计学意义(均P<0.05)。SLeX和Maspin表达与年龄无关(均P>0.05)。在TNM(T3+T4)期、低分化、有淋巴结宏转移和5年内复发转移病人中SLeX高表达,Maspin低表达,呈负相关(r=-0.33)。结论SLeX和Maspin与BIDC发生发展显著相关,SLeX表达上调或(和)Maspin表达下调均预示癌组织分化差、进展快、预后不良。Objective To investigate the SLeX and Maspin expression in breast invasive ductal carcinoma(BIDC)and the relationship with clinicopathologic parameters and prognosis.Methods A retrospective study of 80 continuous patients with BIDC who underwent resection was conducted.The expression of SLeX and Maspin in 80 BIDC tissues and 30 paracancerous tissues were detected by EliVision^(TM) plus two-step immunohistochemical method.The relationships between the expressions of SLeX and Maspin and the clinicopathologic parameters including patient age,TNM staging,differentiated degree,lymph node macrometastasis,recurrence and metastasis 5 years after surgery were studied.Results The positive rates of SLeX and Maspin were 86.3%and 47.5%for cancer tissues and 20.0%and 93.3%for paracancerous tissues,respectively,with statistically significant differences(P<0.05).There was no significant difference in the expressions of SLeX and Maspin between the age of two groups(P>0.05).In patients with high TNM stage(T3 and T4),poor differentiation,lymph node macrometastasis,and recurrence and metastasis within 5 years of follow-up,SLeX was highly expressed,but Maspin was low expressed.A significant inverse relationship was observed between the expressions of SLeX and Maspin in BIDC tissue(r=-0.33).Conclusion Both expressions of SLeX and Maspin may take part in the progress of BIDC.The up-regulated SLeX expression and the down-regulated Maspin expression may hint poor differentiation,rapid progress and poor prognosis of carcinoma tissue.
