特征性引导运动想象对脑卒中偏瘫患者焦虑抑郁及残疾接受度的影响

摘　　要：目的观察特征性引导运动想象训练对脑卒中偏瘫患者焦虑、抑郁及残疾接受度的影响。方法于2018年10月~2019年5月选取河北省保定市泰和康复医院住院的脑卒中偏瘫患者70例为研究对象,焦虑、抑郁评分≥8分。随机分为观察组和对照组。对照组给予神经外科常规康复治疗及护理,观察组在此基础上增加特征性引导运动想象训练。分别于干预前、干预4周后应用医院焦虑抑郁量表(HADS)、残疾接受度量表(ADS-R)评测,比较疗效。结果干预4周后,观察组焦虑得分为(9.66±1.589)分、抑郁得分为(10.06±1.626)分,总分为(19.71±2.191)分,均明显低于对照组,差异有统计学意义(P<0.05);ADS-R扩大维度得分为(23.60±1.499)分、从属维度得分为(12.91±1.095)分、转变维度得分为(22.11±1.451)分,总分为(81.11±3.437)分,均明显高于对照组,差异有统计学意义(P<0.05)。结论特征性引导运动想象训练可有效改善脑卒中偏瘫患者焦虑、抑郁及残疾接受度。Objective To observe the effect of characteristic guided motor imagery training on anxiety,depression and disability acceptance of stroke patients with hemiplegia.Methods From October 2018 to May 2019,70 stroke hemiplegic patients hospitalized in Taihe rehabilitation hospital of Baoding City,Hebei Province with anxiety and depression≥8 points after had evaluation were randomly divided into two groups observation group and control group with 36 people in each group.The control group was given routine rehabilitation and nursing of hospital neurosurgery,and the observation group was added with characteristic guided motor imagination training.The effects were evaluated by hospital anxiety and Depression Scale(HADS)and disability Acceptance Scale(ADS-R)before and 4 weeks after the intervention.Results After 4 weeks of intervention,the scores of anxiety(9.66±1.589),depression(10.06±1.626)and total score(19.71±2.191)in the observation group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).The expanded dimension(23.60±1.499),subordinate dimension(12.91±1.095),transformation dimension(22.11±1.451)and total score(81.11±3.437)of ads-r were significantly higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Characteristic guided motor imagination training can effectively improve the acceptance of anxiety,depression and disability in stroke patients with hemiplegia.
