摘  要:目的应用MRI扩散峰度成像(DKI)探讨脑白质高信号(WMH)伴轻度认知功能障碍(MCI)患者扣带束微结构的变化。方法连续收集民航总医院和首都医科大学附属北京朝阳医院年龄≥60岁的中重度WMH患者47例为WMH组,又根据蒙特利尔认知评估量表(MoCA)评分分为伴MCI组30例和无MCI组17例。选择同期体检且无WMH和无MCI者21例为对照组。均进行MRI检查,选择扣带束为感兴趣区,测量DKI参数并进行组间比较,应用Bonferroni方法校正P值,应用Spearman相关性分析。结果WMH组MoCA评分较对照组明显降低(P=0.01)。伴MCI组左侧扣带回部扣带束各向异性分数(FA)、平均峰度(MK)、径向峰度(RK)显著低于对照组(P_(校正)<0.0167);左侧海马部扣带束轴向峰度(AK)、MK、RK显著低于对照组(P_(校正)<0.0167)。右、左侧扣带回部扣带束FA(r=0.34,0.35)、AK(r=0.35,0.41)、MK(r=0.38,0.44)、RK(r=0.37,0.42)与MoCA评分呈正相关(P<0.01)。结论WMH与MCI相关,DKI技术可评估WMH伴MCI患者扣带束微结构的变化。Objective To study the microstructural changes of singulate bundle in white matter hyperintensities(WMH)patients with MCI by MRI diffusion kurtosis imaging(DKI).Methods Forty-seven≥60 years old patients with moderate-severe WMH served as a WMH group.The patients were further divided into WMH with MCI group(n=30)and WMH without MCI group(n=17)with 21 subjects with no WMH and MCI detected in physical examination during the same period served as a control group.The patients in WMH with MCI group(n=30),WMH without MCI group and the subjects in control group underwent MRI.The DKI parameters of cingulum selected as a region of interest were compared between different groups.The P value was adjusted using Bonferroni method.The correlation between microstructural changes of cingulate bunble in WMH patients with MCI was analyzed by Spearman correlation analysis.Results The MoCA score was significantly lower in WMH group than in control group(P=0.01).The FA,MK and RK in singulate gyrus bundle of left cingulum and left hippocampus were significantly lower in WMH with MCI group than in control group(P<0.0167).The FA,AK,MK and RK in cingulate gyrus bundles of bilateral siguli were positively related with MoCA score in WMH patients with MCI(P<0.01).Conclusion WMH are related with MCI.DKI technique can assess the microstructural changes of cingulate bunble in WMH patients with MCI.
