摘　　要：目的:分析重组人组织型纤溶酶原激活剂(rt-PA)溶栓联合依达拉奉治疗急性缺血性脑卒中的临床效果。方法:回顾性分析广州医科大学附属第六医院2017年2月-2019年2月收治的80例急性缺血性脑卒中患者的临床资料,根据治疗方法分为对照组(40例,采纳rt-PA溶栓治疗)、观察组(40例,采用rt-PA溶栓+依达拉奉治疗),两组均治疗2周。对比两组临床疗效、血流动力学指标、神经功能及出血事件发生率。结果:观察组临床总有效率(95.00%)明显比对照组(72.50%)高,观察组治疗2周后脑组织血管阻力指数(RI)显著比对照组低,血流最大峰值流速(Vs)、平均流速(Vm)显著比对照组快,差异均有统计学意义(P<0.05)。治疗2周后两组血清基质金属蛋白酶-9(MMP-9)、胶质纤维酸性蛋白(GFAP)及美国国立卫生研究院卒中量表(NIHSS)评分均显著降低,差异有统计学意义(P<0.05);治疗后,观察组MMP-9、GFAP及NIHSS评分均明显低于对照组,差异有统计学意义(P<0.05)。两组出血事件发生率比较,差异无统计学意义(P>0.05)。结论:rt-PA溶栓联合依达拉奉可有效减轻急性缺血性脑卒中患者神经功能受损情况,降低血管阻力,安全性高,值得借鉴。Objective:To analyze the clinical efficacy of Recombinant Human Tissue-type Plasminogen Activator(rt-PA)combined with Edaravone in the treatment of acute ischemic stroke.Method:The clinical data of eighty patients with acute ischemic stroke admitted to the Sixth Affiliated Hospital of Guangzhou Medical University from February 2017 to February 2019 were retrospectively analyzed.They were divided into control group(40 cases with rt-PA thrombolytic therapy),observation group(40 cases with rt-PA thrombolysis+Edaravone treatment)according to the treatment method,both groups were treated for 2 weeks.The clinical efficacy,hemodynamics indexes,neurological function and the incidence of bleeding events were compared between the two groups.Result:The total clinical effective rate(95.00%)of the observation group was significantly higher than 72.50% of the control group;after 2 weeks of treatment,the cerebral vascular resistance index(RI)of the observation group was significantly lower than that of the control group,and the maximum peak velocity(Vs)and mean velocity(Vm)of blood flow were significantly faster than those of the control group(P<0.05).After two weeks of treatment,the levels of serum matrix metalloproteinase-9(MMP-9),glial fibrillary acidic protein(GFAP)and national institutes of health stroke scale(NIHSS)score of the two groups were significantly reduced,and the differences were statistically significant(P<0.05).After treatment,the levels of MMP-9,GFAP and NIHSS score of the observation group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of bleeding between the two groups(P>0.05).Conclusion:rt-PA thrombolysis combined with Edaravone can effectively reduce the neurological impairment in patients with acute ischemic stroke,reduce vascular resistance,and have higher safety and worthy of reference.
