丁苯酞联合胞磷胆碱治疗血管性痴呆的效果及对患者认知功能、血清Livin及IL-18水平的影响

摘　　要：目的分析丁苯酞联合胞磷胆碱治疗血管性痴呆(VD)的效果及对患者认知功能、血清抗凋亡因子(Livin)及白细胞介素-18(IL-18)水平的影响。方法抽取2017年7月至2020年7月无锡市人民医院收治的68例VD患者作为研究对象,根据随机数字表法分为对照组与观察组,每组34例。对照组采用胞磷胆碱治疗,观察组采用丁苯酞与胞磷胆碱联合治疗。比较两组患者的临床疗效、简易智力状态检查量表(MMSE)评分、血清Livin与IL-18水平。结果治疗后,两组患者MMSE各项评分及总分增加,观察组患者MMSE各项评分及总分高于对照组(P<0.05)。治疗后,两组总有效率比较,P>0.05;但观察组疗效等级优于对照组(P<0.05)。治疗后,两组患者血清Livin水平上升,IL-18水平下降,观察组血清Livin水平高于对照组,IL-18水平低于对照组(P<0.05)。结论丁苯酞、胞磷胆碱联合治疗VD患者,可以进一步提升疗效,恢复患者正常认知功能,同时调节其体内血清Livin及IL-18水平。Objective To analyze the effects of butylphthalide combined with citicoline on vascular dementia(VD)and its influence on cognitive function,serum anti-apoptosis factor(Livin)and interleukin-18(IL-18)levels.Methods A total of 68 patients with VD who were diagnosed and treated in Wuxi People’s Hospital from July 2017 and July 2020 were selected as the research subjects between.According to the random number table method,the patients were divided into control group and observation group,with 34 cases in each group.Patients in the control group were treated with citicoline,while patients in the observation group were treated with butylphthalide combined with citicoline.The mini-mental state examination(MMSE)scores,clinical effects,serum Livin and IL-18 levels were compared between the two groups.Results After treatment,the MMSE scores and total scores of the two groups were increased,those in the observation group were higher than those in the control group(P<0.05).After treatment,the total response rate was compared between the two groups,P>0.05;but the grade of curative effect in observation group was better than that in the control group(P<0.05).After treatment,serum Livin level was increased while IL-18 level was reduced in the two groups.Meanwhile,the serum Livin level of the observation group was higher than that of the control group,and the IL-18 level was lower than that of the control group(P<0.05).Conclusions The combined treatment of butylphthalide and citicoline for patients with VD can enhance the curative effect,restore the patients’cognitive function,regulate serum Livin and IL-18 levels.
