玻璃酸钠联合rhEGF滴眼液治疗白内障术后干眼症的临床效果

摘　　要：目的探讨玻璃酸钠联合重组人表皮生长因子(rhEGF)滴眼液治疗白内障术后干眼症的临床效果。方法抽取2019年11月至2020年10月郑州市第二人民医院收治的白内障术后干眼症患者90例。根据用药方案不同分为玻璃酸钠治疗组和联合rhEGF治疗组,每组45例。比较两组患者的临床疗效,治疗前后的泪膜破裂时间(BUT)、泪液分泌长度、角膜荧光素染色(FL),以及白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)水平。结果治疗8周后,联合rhEGF治疗组患者临床总有效率(97.78%)高于玻璃酸钠治疗组的84.44%(P<0.05)。治疗8周后,联合rhEGF治疗组患者的BUT高于玻璃酸钠治疗组,而泪液分泌长度和FL低于玻璃酸钠治疗组(P<0.05),联合rhEGF治疗组患者的IL-6和TNF-α水平低于玻璃酸钠治疗组(P<0.05)。结论玻璃酸钠联合rhEGF治疗白内障术后干眼症具有较好的临床疗效,可延长BUT,减少泪液分泌长度和FL,降低泪液中炎症因子IL-6和TNF-α水平,从而达到稳定泪膜、平衡泪液分泌、修复损伤角膜的作用。Objective To investigate the clinical effect of sodium hyaluronate combined with recombinant human epidermal growth factor(rhEGF)eye drops in the treatment of dry eye disease after cataract surgery.Methods Ninety patients with dry eye disease after cataract surgery in Zhengzhou Second People’s Hospital from November 2019 to October 2020 were selected.According to different medication schemes,they were divided into sodium hyaluronate treatment group and rhEGF combined group.The clinical efficacy,tear film break-up time(BUT),tear secretion length,fluorescent staining(FL),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)levels before and after treatment were compared between sodium hyaluronate treatment group and rhEGF combined group.Results After 8 weeks of treatment,the total clinical effective rate in rhEGF combined group(97.78%)was higher than 84.44%in sodium hyaluronate treatment group(P<0.05).After 8 weeks of treatment,the BUT in rhEGF combined group was higher than that in the sodium hyaluronate treatment group,while the tear secretion length and FL were higher than those in sodium hyaluronate treatment group(P<0.05),and the levels of IL-6 and TNF-αin rhEGF combined group were lower than those in sodium hyaluronate treatment group(P<0.05).Conclusions Sodium hyaluronate combined with rhEGF has a good clinical effect in treatment of dry eye after cataract surgery,which can significantly prolong BUT,reduce tear secretion length and FL,and reduce the levels of inflammatory factors IL-6 and TNF-αin tears,so as to stabilize tear film,balance tear secretion and repair damaged cornea.
