冰硼散加减外用联合穴位注射治疗原发性肝癌中重度疼痛的临床疗效

摘　　要：目的观察冰硼散加减外用联合穴位注射治疗原发性肝癌(PHC)中重度疼痛的临床疗效。方法选取2019年5月-2021年5月建昌县人民医院收治的68例PHC中重度疼痛患者,按照随机数字表法分为对照组和观察组,各34例。对照组给予盐酸羟考酮缓释片治疗,观察组在其基础上应用冰硼散加减外用联合穴位注射治疗,比较两组镇痛效果、疼痛程度数字评估量表(NRS)评分、匹兹堡睡眠质量指数(PSQI)评分、前列腺素E2(PGE2)、缓激肽(BK)、β-内啡肽(β-ENP)及不良反应。结果观察组镇痛有效率为91.18%,高于对照组的70.59%(P<0.05);两组NRS、PSQI评分低于治疗前,且观察组低于对照组(P<0.05);两组PGE2、BK低于治疗前,β-ENP高于治疗前,且观察组PGE2、BK低于对照组,β-ENP高于对照组(P<0.05);观察组不良反应发生率为29.41%,低于对照组的52.94%(P<0.05)。结论冰硼散加减外用联合穴位注射治疗PHC中重度疼痛效果确切,可有效减轻其癌性疼痛,改善睡眠质量,降低肿瘤细胞炎性因子分泌,减少药物不良反应。Objective To observe the clinical efficacy of modified Bingpeng powder combined with acupoint injection in the treatment of moderate to severe pain in primary hepatic carcinoma(PHC).Methods A total of 68 patients with moderate to severe pain of PHC admitted to Jianchang County People's Hospital from May 2019 to May 2021 were selected and randomly divided into control group and observation group,with 34 cases in each group.The control group was treated with oxycodone hydrochloride sustained release tablets,and the observation group was treated with Bingpeng powder combined with acupoint injection.The analgesic effect,NRS score,Pittsburgh sleep quality index(PSQI)score,prostaglandin E2(PGE2),bradykinin(BK),β-endorphin(β-ENP)and adverse reactions were compared between the two groups.Results The effective rate of analgesia in the observation group was 91.18%,which was higher than 70.59%in the control group(P<0.05).The NRS and PSQI scores of the two groups were lower than those before treatment,and the observation group was lower than the control group(P<0.05).PGE2 and BK in the two groups were lower than those before treatment,andβ-ENP was higher than that before treatment,PGE2 and BK in the observation group were lower than those in the control group,andβ-ENP was higher than that in the control group(P<0.05).The incidence of adverse reactions in the observation group was 29.41%,which was lower than 52.94%in the control group(P<0.05).Conclusion Modified Bingpeng powder combined with acupoint injection is effective in the treatment of moderate and severe pain in PHC,which can effectively relieve cancer pain,improve sleep quality,reduce the secretion of inflammatory factors in tumor cells and reduce adverse drug reactions.
