用保留喉功能的咽喉癌切除术治疗咽喉癌的临床效果

摘　　要：目的:评估咽喉癌患者应用保留喉功能的咽喉癌切除术的疗效。方法:选择兰陵县人民医院在2017年6月-2021年6月接诊的60例咽喉癌患者,根据随机数字表法进行分组,对照组与观察组各30例,对照组给予不保留喉功能的咽喉癌切除术,观察组给予保留喉功能的咽喉癌切除术,将两组咽喉癌患者的治疗效果进行比较。结果:观察组术后吞咽功能恢复正常率(96.67%)与对照组的手术效果比较,差异无统计学意义(P>0.05)。观察组并发症(感染性咽瘘、颈部皮下感染、胃部出血、肺部感染)发生率(20.00%)低于对照组,差异均有统计学意义(P<0.05)。观察组焦虑自评量表评分(36.65±2.51)分,抑郁自评量表评分(34.59±3.52)分,分别低于对照组的(34.59±3.52)分、(40.15±4.15)分,差异均有统计学意义(P<0.05)。观察组健康调查简表(SF-36)生活质量评分、生理功能评分、社会功能评分、精神健康评分和躯体疼痛评分均高于对照组,差异有统计学意义(P<0.05)。结论:咽喉癌患者应用保留喉功能的咽喉癌切除术的效果显著,值得在临床中推广使用。Objective:To evaluate the efficacy of laryngeal carcinoma resection with laryngeal function preserved.Methods:Sixty patients with laryngeal carcinoma admitted in Lanling People’s Hospital from June 2017 to June 2021 were selected,according to random number table method they were divided into control group and observation group with 30 cases in each group.The control group was given laryngeal carcinoma resection without laryngeal function preserved,the observation group was given laryngeal carcinoma resection with laryngeal function preserved.The treatment effect of patients with laryngeal carcinoma in two groups was compared.Results:The normal rate of swallowing function in the observation group(96.67%)was not significantly different from that in the control group after surgery(P>0.05).The incidence of complications(infectious pharyngeal fistula,neck subcutaneous infection,stomach bleeding,lung infection)in the observation group(20.00%)was lower than that in the control group,with statistically significant difference(P<0.05).The scores of self-rating anxiety scale score(36.65±2.51)and self-rating depression scale score(34.59±3.52)in the observation group were lower than those in the control group(34.59±3.52)and(40.15±4.15),with statistically significant difference(P<0.05).The Health Survey Short Form(SF-36)quality of life score,physiological function score,social function score,mental health score and physical pain score in the observation group were higher than those in the control group,with statistically significant difference(P<0.05).Conclusion:Laryngeal carcinoma resection with laryngeal function preserved has a significant effect on patients with laryngeal carcinoma,which is worth popularizing in clinic.
