摘 要:目的评价尿液中期因子(Midkine,MK)作为膀胱癌非侵入性诊断指标的应用价值。方法收集64例膀胱癌患者、49例良性泌尿系统疾病患者和52例健康体检者的尿液,应用ELISA法检测尿液MK浓度,同时以尿液肌酐(Creatinine,Cr)浓度对MK进行定量标准化,比较3组研究对象MK浓度差异,并绘制受试者工作特征曲线(receiver operating characteristic curve,ROC),评估其对膀胱癌诊断的灵敏度、特异度并计算曲线下面积(Area under curve,AUC)。同时比较不同尿液MK水平与膀胱癌患者的临床和病理特征的相关性。结果膀胱癌患者尿液中MK浓度为0.91(0.58,1.23)ng/mg Cr;良性泌尿系统疾病患者中尿液MK浓度为0.53(0.41,0.58)ng/mg Cr;健康体检者尿液MK浓度为0.24(0.17,0.37)ng/mg Cr,前者尿液MK水平显著高于后两者,差异有统计学意义(P<0.05)。尿液MK诊断膀胱癌的ROC曲线下面积AUC为0.87,诊断的灵敏度和特异度分别为71.9%和92.1%,约登指数为0.64。尿液MK水平与肿瘤数目、侵袭性、是否发生淋巴结转移密切相关(P<0.05)。结论尿液MK检测可能成为膀胱癌的非侵入性诊断标志物,并且可能成为预后预测指标。Objective To evaluate the application value of urinary midkine(MK)as a non-invasive diagnostic marker of bladder cancer.Methods Urine samples were collected from 64 patients with bladder cancer,49 patients with benign urinary diseases and 52 healthy controls.The concentration of urinary MK was detected by enzyme-linked immunosorbent assay(ELISA),urine MK level was quantitatively standardized by urine creatinine(Cr)concentration.The differences of urine MK level among the three groups were compared,and the receiver operating characteristic curve(ROC)was drawn to evaluate its sensitivity and specificity for the diagnosis of bladder cancer and calculate the area under the curve(AUC).Meanwhile,the correlation between clinical and pathological features of bladder cancer patients with different urine MK levels was compared.Results The urine MK level of patients with bladder cancer was 0.91(0.58,1.23)ng/mg Cr,the MK levels of patients with benign urinary diseases and healthy controls were 0.53(0.41,0.58)ng/mg Cr and 0.24(0.17,0.37)ng/mg Cr,respectively.The urine MK level of the former was significantly higher than that of the latter two,and the difference was statistically significant(P<0.05).The area under the curve(AUC)of urine MK for the diagnosis of bladder cancer was 0.87,and the sensitivity and specificity of diagnosis were 71.9%and 92.1%,respectively.The Youden index was 0.64.The level of MK was closely related to the number of tumors,invasiveness and lymph node metastasis(P<0.05).Conclusion Urine MK might be a non-invasive diagnostic marker of bladder cancer and may be related to the poor prognosis of bladder cancer.
