2015—2020年某院门诊二类精神药品处方分析

摘　　要：目的了解某院门诊二类精神药品的应用情况,为规范精神类药品的管理和应用提供可靠、有效的依据。方法收集2015年10月至2020年3月某院门诊的二类精神药品的处方信息,统计药品名称、规格、数量等,对药品的用药频度(defined daily dose system,DDDs)、药物利用指数(drug utilization index,DUI)等进行计算和分析比较。结果某院门诊苯二氮艹卓类药品的处方数量最高,占总处方量的81.25%,单药品处方量中,阿普唑仑、氯硝西泮和劳拉西泮处方量排在前三位;劳拉西泮、艾司唑仑的DUI均大于1.0,提示个别患者存在用药不合理现象;阿普唑仑、劳拉西泮和佐匹克隆片三种药物连续四年B/A均大于1.0。结论二类精神药品在该院门诊的应用基本合理,但存在一些不合理用药现象,提示临床医师在选择用药时应综合考虑,减少患者药物滥用和药物依赖性。Objective To understand the application of Class II psychotropic drugs in outpatient department of one hospital,and to provide reliable and effective basis for standardizing the management and application of psychotropic drugs.Methods We collected the prescription information of the ClassⅡpsychotropic drugs in the hospital from October 2015 to March 2020,counted the names,specifications and quantities of drugs,and analyzed the DDDs(defined daily dose system)and DUI(drug utilization index).Results The number of prescriptions for outpatient benzodiazepines in the hospital was the highest,accounting for 81.25%of the total prescriptions.Among the prescriptions for single drugs,the prescriptions for alprazolam,clonazepam,and lorazepam ranked the top three.The DUI of lorazepam and estazolam were both greater than 1.0,and the medication was unreasonable;the B/A of alprazolam,lorazepam and zopiclone tartrate were all greater than 1.0 for fourconsecutive years.Conclusion The use of Class II psychotropic drugs in the outpatient department of the hospital is basically reasonable,but some drugs also have some unreasonable phenomena.It is recommended that clinicians should consider comprehensively when choosing drugs to reduce patients′dependence on drugs.
