摘　　要：目的 超声检测精神分裂症患者颈动脉内-中膜的影像改变,分析第二代抗精神病药物的代谢综合征与患者颈动脉内-中膜(IMT)的相关性。方法 选取我院服用奥氮平、利培酮、喹硫平、齐拉西酮、阿立哌唑等第二代抗精神病药物10年以上,年龄在30岁以上的120例慢性精神分裂症患者作为研究组。选取本院颈动脉体检的职工,年龄相当,作为对照组。应用血管超声技术测量双侧颈总动脉、球部及右侧锁骨下动脉(RScA)管腔内-中膜厚度;采集精神分裂症患者代谢综合征数据,包括体重指数(BMI)、甘油三酯(TG)、胆固醇(TC)、血糖(FBG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)。对检测结果分析。结果 研究组代谢综合征腰围、BMI、FBG、TG、TC、HDL-C、LDL-C明显高于对照组,研究组超声监测颈动脉内-中膜(IMT)值显著高于对照组(P<0.05),颈动脉硬化与斑块发生率也高于对照组(P<0.05),并且代谢综合征数据增加,颈动脉内中膜值也增厚,两者呈正相关(r=0.024-0.238,P<0.05)。结论 长期服用第二代抗精神病药物导致的代谢综合征(MS)与精神分裂症患者颈动脉内-中膜(IMT)增厚形成呈正相关,超声检测颈动脉内中膜厚度可以引导临床,合理使用药物和配比,提高患者生存质量。Objective The relationship between the Metablic Syndrome of the Second Genneration antipsychotics and the Carotid intima media in schizophrenia patuents was analyzed. Methods The study group consisted of 120 patients over 30 years of age who had been taking OLanzapine,risperidone,quetiapine,Ziprassidone,Aripiprazole,etc. For antipsychotic. Select the carotid physical examination of the hospital staff,the same age,as a control group.The media thickness of the commom carotid artery,the Bulbar and the right subclavian artery lumen was measured by vascular ultrasound,the results of body mass index,TG,TC,Blood Glucose,LDL-C and HDL-C were analyzed. Results The metabolic synthesis of waist,BMI、FBG、TG、TC、HDL-C and LDL-C in the experimental group were significantly higher than those in the control group. The IMT of carotid artery in the study group was significantly higher than that in the control group(P<0.05),and the incidence of carotid arteriosclerosis and plaqe was also higher than that in the control group(P<0.05),the two are positively correlated(r=0.024-0.238,P<0.05).Conclusion Long-term administration of the second-generation anti-disease drug delivery system(MS)was associated with the thickening of IMT in the recalcitrant arteries of patients with mental decibel,improve patient suivival,quality conteol.
