舍曲林联合认知行为治疗对产妇产后抑郁症的效果

摘　　要：目的 分析产妇产后抑郁症(PPD)接受舍曲林联合认知行为治疗的效果。方法 将2018年11月~2019年11月接诊且行舍曲林治疗的46例PPD产妇作为对照组,将2019年12月~2020年12月接诊且在对照组基础上加用认知行为治疗的46例PPD产妇作为观察组,比较两组的睡眠质量、认知功能、产后抑郁状态、社会功能、临床疗效。结果 (1)治疗后观察组睡眠质量评分更低(P<0.05);(2)两组患者的认知功能、产后抑郁状态、社会功能评分在产后14天时无明显差异(P>0.05);观察组产后42天、产后3个月时的认知功能、产后抑郁状态、社会功能评分均优于对照组(P<0.05);(3)观察组3例无效(6.52%),对照组10例无效(21.74%)(P<0.05)。结论对PPD产妇实施舍曲林+认知行为治疗,可以改善产妇睡眠质量、抑郁状态、认知及社会功能,疗效显著,值得推广。Objective To analyze the effect of sertraline combined with cognitive behavioral therapy on postpartum depression(PPD). Methods 46 cases of PPD puerpera treated with sertraline from November 2018 to November 2019 were selected as the control group,and 46 cases of PPD puerpera treated with cognitive behavioral therapy on the basis of the control group from December 2019 to December 2020 were selected as the observation group. The sleep quality,cognitive function,postpartum depression,social function and clinical efficacy of two groups were compared.Results (1)After treatment,the sleep quality score of the observation group was lower,P<0.05.(2)There was no significant difference in the scores of cognitive function,postpartum depression and social function between the two groups at 14 days postpartum,P>0.05. The scores of cognitive function,postpartum depression and social function of the observation group at 42 days postpartum and 3 months postpartum were better than those of the control group,P<0.05.(3)three cases in the observation group were ineffective(6.52%),while 10 cases were ineffective(21.74%)in the control group,P<0.05. Conclusion Sertraline combined with cognitive behavior therapy can improve the sleep quality,depression,cognitive and social function of PPD puerpera,and has significant effect.
