淋巴管肌瘤病与肝硬化相关乳糜腹淋巴循环异常的影像学比较研究

摘　　要：目的比较淋巴管肌瘤病(LAM)和肝硬化相关乳糜腹淋巴循环异常的影像学特点。方法回顾性分析19例LAM相关乳糜腹和22例肝硬化相关乳糜腹的直接淋巴管造影(DLG)和CT淋巴管成像(CTL)资料,比较两组的淋巴循环异常影像特征。结果与肝硬化相关乳糜腹相比,LAM相关乳糜腹更易出现造影对侧髂腰部的对比剂异常反流(68.4%vs.18.2%,P=0.002)和盆腔周围淋巴管扩张(26.3%vs.0,P=0.016),腹膜后或髂充盈对比剂的团状淋巴结构最大径更宽[(2.08±1.03)cm vs.(0.95±0.21)cm,P<0.001]。结论 LAM和肝硬化两种病因相关乳糜腹的淋巴循环呈现不同的影像学特点。Objective To compare the abnormal lymphatic features of lymphangioleiomyomatosis with chyloperitoneum and cirrhosis-related chylous ascites. Methods The imaging data of direct lymphangiography and CT lymphangiography for 19 cases of lymphangioleiomyomatosis with chyloperitoneum and 22 cases with cirrhosis-related chylous ascites were retrospectively analyzed for comparing the imaging characteristic of lymphatic abnormalities. Results Compared with cirrhotic chyloperitoneum, lymphangioleiomyomatosis related chylous ascites was more prone to abnormal contrast medium reflux(68.4% vs 18.2%,P= 0.002),peripelvic lymphangiectasia(26.3% vs 0,P= 0.016),and the wider maximum diameter of the ipsilateral injection side of retroperitoneal or iliac lymphatic cluster structure filled with contrast medium [(2.08±1.03)cm vs(0.95±0.21) cm, P<0.001]. Conclusion The abnormal lymphatic circulation features are different in chyloperitoneum of lymphangioleiomyomatosis and cirrhotic chylous ascites.
