摘  要:目的了解北京市18~65岁居民肥胖、中心性肥胖及睡眠情况,探索睡眠时间与肥胖、中心性肥胖的关系,为制定完善慢性病相关防控策略与措施提供科学依据。方法数据来自2017年北京市成人慢性病及其危险因素监测资料,该监测于2017年8—12月,采取多阶段分层抽样方法在北京市16个区抽取18~79岁居民为调查对象进行问卷调查、体格测量和实验室检测。该研究以其中11605名18~65岁居民作为研究对象。采用SPSS 19.0软件进行χ^(2)检验、Wilcoxon秩和检验、Kruskal-Wallis H秩和检验和趋势χ^(2)检验,采用多因素logistic回归分析睡眠时间对肥胖和中心性肥胖的影响。结果纳入有效样本共11605人,肥胖率为24.01%,中心性肥胖率为41.53%,平均睡眠时间为(7.51±1.26)h,睡眠不足比例为19.50%,睡眠过多比例为15.55%。<7、7~8和>8 h睡眠时间者的肥胖率分别为27.09%、22.86%和24.94%,差异有统计学意义(χ^(2)=18.103,P<0.01)。<7、7~8和>8 h睡眠时间者的中心性肥胖率分别为45.82%、40.05%和42.29%,差异有统计学意义(χ^(2)=24.416,P<0.01)。调查对象肥胖率与中心性肥胖率均随睡眠时间增加先降低后升高。多因素logistic回归分析结果显示,相比7~8 h睡眠者,睡眠时间<7 h(OR=1.23,95%CI:1.10~1.37)易发生肥胖;睡眠时间<7 h(OR=1.11,95%CI:1.00~1.22)和睡眠时间>8 h(OR=1.14,95%CI:1.03~1.27)易发生中心性肥胖。结论北京市18~65岁居民存在睡眠时间不足与过多问题,睡眠时间是肥胖与中心性肥胖的影响因素。Objective To understand the obesity,central obesity and sleeping status among residents(18-65 years old)in Beijing,explore the relationship between obesity or central obesity and sleeping duration and provide the scientific reference for formulating and improving the relevant prevention and control strategies and measures.Methods The data were from Chronic Disease Risk Factor Surveillance in Beijing in 2017.The surveillance was conducted from August to December 2017.Residents(18-79 years old)were selected from 16 districts of Beijing by multi-stage stratified method,and the investigation was performed with the questionnaire,physical examination and laboratory tests.In this study,11605 residents(18-65 years old)were selected as the objects.Wilcoxon test,Kruskal-Wallis H test,χ^(2) test and trendχ^(2) test were used to analyze the data,the multivariate logistic regression was used to analyze the influence of sleeping duration on obesity or central obesity.Results Among 11605 participants,the morbidities of obesity and central obesity were 24.01%and 41.53%,respectively;the average sleeping duration was(7.51±1.26)hours,the rates of insufficient and excessive sleep duration were 19.50%and 15.55%,respectively.The obesity rates in subjects with<7,7-8 and>8 h sleeping duration were 27.09%,22.86%and 24.94%,respectively(χ^(2)=18.103,P<0.01).The central obesity rates in subjects with<7,7-8 and>8 h sleeping duration were 45.82%,40.05%and 42.29%,respectively(χ^(2)=24.416,P<0.01).The morbidities of obesity and central obesity decreased with sleeping duration,then increased.The multivariate logistic regression showed that as compared with the subjects with 7-8 h sleeping duration,the subjects with<7 h sleeping duration suffered with obesity(OR=1.23,95%CI:1.10-1.37),the subjects with<7 and>8 h sleeping duration suffered with central obesity(OR=1.11,95%CI:1.00-1.22;OR=1.14,95%CI:1.03-1.27).Conclusion There is the problem of insufficient and excessive sleep duration in residents(18-65 years old)of Beijing,the sleeping duratio
