ω-3多不饱和脂肪酸辅助治疗溃疡性结肠炎的效果及其对血清Th1和Th2细胞因子水平的影响

摘　　要：目的探究溃疡性结肠炎在常规美沙拉嗪治疗的基础上加用ω-3多不饱和脂肪酸(ω-3PUFA)的治疗效果,以及对患者血清Th1及Th2细胞因子水平的影响。方法选取2019年1月—2021年1月在鞍山市中心医院接受治疗的120例溃疡性结肠炎患者,随机分为对照组和观察组各60例。对照组给予美沙拉嗪治疗,观察组给予ω-3PUFA联合美沙拉嗪治疗,两组均连续治疗2周,比较两组的临床疗效,治疗前后临床症状评分、病变活动程度评分、血清Th1及Th2细胞因子水平,以及两组不良反应发生情况。结果观察组临床总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组临床症状评分和病变活动程度评分均低于对照组,差异有统计学意义(P<0.05);观察组IL-4水平较对照组低,IL-10和INF-γ水平较对照组高,差异有统计学意义(P<0.05);两组IL-2水平比较,差异无统计学意义(P>0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论对于溃疡性结肠炎患者的临床治疗,在常规美沙拉嗪治疗基础上加用ω-3PUFA能够提高疗效,进一步改善和缓解患者的临床症状及体征,降低病变活动性,同时可调节血清Th1及Th2细胞因子水平,且治疗期间未增加不良反应,值得临床推广应用。Objective To explore the therapeutic effect of ω-3 PUFA on the basis of conventional mesalazine in the treatment of ulcerative colitis and its effect on the levels of serum Th1 and Th2 cytokines.Methods A total of 120 patients with ulcerative colitis treated in Anshan Central Hospital from January 2019 to January 2021 were randomly divided into control group and observation group.The control group was treated with mesalazine,and the observation group was treated with ω-3 pufa combined with mesalazine.Both groups were treated for 2 weeks.The clinical efficacy,clinical symptom score,lesion activity score,serum Th1 and Th2 cytokine levels before and after treatment were compared between the two groups.The incidence of adverse reactions during treatment was compared between the two groups.Results The total clinical effective rate of the observation group was significantly higher than that of the control group(P<0.05).After treatment,the scores of clinical symptoms and lesion activity in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the level of IL-4 in the observation group was significantly lower than that in the control group,while IL-10 and INF-γ levels were significantly higher than those in the control group(P<0.05).There was no significant difference in the level of IL-2 between the two groups after treatment(P>0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion For the clinical treatment of patients with ulcerative colitis,ω-3 PUFA addition on the basis of conventional mesalazine treatment can improve the curative effect,improve and alleviate the clinical symptoms and signs of patients,reduce the activity of lesions,regulate the levels of serum Th1 and Th2 cytokines,and there are no adverse reactions during treatment,which is worthy of promotion.
