血常规LMR、HCT、PCT联合检测评估克罗恩病活动性的价值

摘　　要：目的分析血常规淋巴细胞/单核细胞(LMR)、红细胞压积(HCT)和血小板比容(PCT)联合检测评估克罗恩病(CD)活动性的价值。方法选择医院2019年2月-2020年2月诊治的成年CD患者96例,将其纳入病例组。另选取同期体检健康的96例受试者,将其纳入健康组。检测血常规LMR、HCT、PCT水平,评定CD活动性。结果病例组患者的LMR(3.14±0.12)低于健康组(P<0.05),HCT(47.22±3.57)、 PCT(13.58±1.21)水平高于对照组(P<0.05)。随着疾病加重,LMR显著下降,HCT、PCT水平明显上升。轻度、中度和重度患者LMR、HCT、PCT水平差异比较有统计学意义(P<0.05)。根据Spearman相关性分析,将CD的活动程度轻度、中度和重度分别赋值为(轻度=1,中度=2,重度=3),检验得出CD活动程度与LMR与成负相关性为(r=-0.477,P<0.001),CD活动程度与HCT、PCT成正相关性分别为(r=0.671,r=0.643,P<0.001)。结论血常规LMR、HCT、PCT联合检测可以更加明确评估克罗恩病活动性,密切监测有利于病情康复。Objective To investigate the value of combined detection of lymphocyte/monocyte ratio(LMR),hematoctet(HCT)and plateletcrit(PCT)in the evaluation of Crohn′s disease(CD).Methods Ninety-six patients with CD and ninety-seven healthy individuals in our hospital from February 2019 to February 2020 were enrolled, and set as case group and healthy group, respectively.The levels of LMR,HCT and PCT were detected, and the activity of CD was evaluated.Results Compared with healthy group, case group had a lower LMR(3.14±0.12),and a higher levels of HCT(47.22±3.57)and PCT(13.58±1.21),with statistic difference(P<0.05).With the aggravation of CD,LMR was decreased, meanwhile, HCT and PCT were increased significantly.Levels of LMR,HCT and PCT showed no significant difference among CD patients of different severity degrees(P>0.05).After Spearman correlation analysis assigned mild, moderate and severe activities of CD(mild=1,moderate=2,severe=3),the analysis showed that CD activity degree was negatively correlated with LMR(r=-0.477,P<0.001),and positively correlated with HCT and PCT(r=0.671,r=0.643,P<0.001).Conclusion Combined detection of LMR,HCT and PCT can effectively evaluate the activity of CD,moreover, patients with high levels of LMR,HCT and PCT suffer a severe activity of disease.
