摘 要:目的国产康多机器人辅助腹腔镜系统(型号:KD-SR-01)在妇科手术的安全性及有效性。方法将3~4个月30~40 kg雌性长白猪18头随机数字表法分为传统腹腔镜组(n=6)、机器人单孔(R-LESS)组(n=6)和机器人多孔(R-LEMS)组(n=6),行腹腔镜下全子宫切除术,比较3组的总手术时间、对接时间、子宫切除时间、阴道残端缝合时间、术中出血量、中转率及术中并发症情况。组间比较采用单因素方差分析,两两比较采用LSD检验。结果所有手术均顺利完成,无中转开腹,无术中并发症发生,3组出血量均<5 ml。与传统腹腔镜[总手术时间(47.83±8.59)min,子宫切除时间(16.00±3.46)min,阴道残端缝合时间(11.83±1.47)min]比较,R-LEMS组总手术时间[(56.20±6.87)min]及具体操作时间 子宫切除时间[(17.40±4.72)min],阴道残端缝合时间[(15.80±2.86)min]均无明显延长(t=1.693、0.608、2.573,P>0.05)。与R-LEMS组[对接时间(14.20±5.72)min],子宫切除时间[(17.40±4.72)min],总手术时间[(56.20±6.87)min],阴道残端缝合时间[(15.80±2.86)min]比较,R-LESS组对接时间[(10.00±1.90)min]、子宫切除时间[(24.17±7.36)min]差异无统计学意义(t=-1.706、1.990,P>0.05),总手术时间[(67.83±7.41)min]及阴道残端缝合时间[(21.33±4.63)min]更长,差异有统计学意义(t=2.152、2.528,P<0.05)。结论康多腹腔镜手术机器人系统辅助的妇科手术在动物水平安全可行,且单孔可以取得同样满意的效果,但未来仍需加以验证。Objective To evaluate the feasibility and safety of a domestic robotic surgical system(KD-SR-01)for gynecological surgery through an animal experiment.Methods Totally,183-4 months female long white pigs weighing 30-40 kg were randomly divided into conventional laparoscopic group(n=6),robotic laparoendoscopic single-site surgery group(R-LESS)(n=6)and robotic multiple-sites surgery group(R-LEMS)(n=6),and laparoscopic hysterectomy was performed.Total operation time,docking time,hysterectomy time,vaginal stump suture time,intraoperative bleeding,conversion rate and the occurrence of intraoperative complications were recorded.One way ANOVA and LSD test was used.P<0.05 was considered as significant.Results All surgeries were completed successfully,and no conversion to laparotomy or intraoperative complications occurred.The estimated blood loss amount in 3 groups were<5 ml.Compared with the conventional group[total operation time(47.83±8.59)min,hysterectomy time(16.00±3.46)min,vaginal stump suture time(11.83±1.47)min],the total operation time[(56.20±6.87)min]and specific operation time[hysterectomy time(17.40±4.72)min,vaginal stump suture time(15.80±2.86)min]in R-LEMS group were not prolonged(t=1.693,0.608,2.573,P>0.05).The total operation time[(67.83±7.41)min]and vaginal stump suture time[(21.33±4.63)min]were longer in R-LESS group than in R-LEMS group[docking time(14.20±5.72)min,hysterectomy time(17.40±4.72)min,total surgical time(56.20±6.87)min,vaginal stump suture time(15.80±2.86)min,t=2.152,2.528,P<0.05].There was no significant difference in docking time[(10.00±1.90)min]or hysterectomy time[(24.17±7.36)min,t=-1.706,1.990,P>0.05].Conclusion Gynecological surgery assisted by Kangduo robotic surgical system is safe and feasible at the animal level,and the same satisfactory results can be achieved with the single site technique,but it still needed to be verified.
