摘 要:目的探讨鸡颈总动脉血管吻合模型的设计建立及对显微外科初学者血管吻合练习的训练效果。方法采用市场购买的50个长约10 cm的新鲜完整鸡颈, 镜下分离皮下组织, 充分暴露颈总动脉, 同时测量血管长度与口径, 剪断颈总动脉后对断端行端-端吻合, 共计500支。每100个吻合口记为一组, 分A、B、C、D、E共5组, 记录单个血管断端吻合所用时间, 统计每一组所用时间平均值, 通过管腔内注射甲紫溶液检查血管吻合口通畅性。两组间比较采用t检验, 多组间比较采用单因素方差分析。结果本组实验中测量左、右侧颈总动脉长度比较, 差异无统计学意义[左侧动脉长度为(9.57±0.28) cm, 右侧动脉长度为(9.61±0.27) cm, t=0.779, P>0.05];左、右侧头端动脉口径比较, 差异无统计学意义[左侧头端动脉口径为(1.12±0.08) mm, 右侧头端动脉口径为(1.13±0.07) mm, t=0.265, P>0.05];左、右侧尾端动脉口径比较, 差异无统计学意义[左侧尾端动脉口径为(0.71±0.08) mm, 右侧尾端动脉口径为(0.70±0.06) mm, t=0.063, P>0.05]。A~E组单个吻合用时分别为(14.36±0.60)、(12.57±0.71)、(10.07±0.65)、(8.44±0.52)、(6.27±0.74) min, 组间比较差异有统计学意义(F=243.978, P<0.05);A、B组, B、C组, C、D组, D、E组分别比较(t=6.053、8.209、6.178、7.543, P均<0.05), 差异有统计学意义。结论鸡颈总动脉血管吻合模型血管口径粗大, 模型制备简单, 容易暴露, 血管吻合训练效果显著, 是学习血管吻合技术的有效方法。Objective To establish a new vascular anastomosis model of chicken common carotid arteries and to explore the results for novices in the application of vascular anastomosis training.Methods In the microsurgery skills training room of our hospital,50 fresh chicken necks were used and then we separated the subcutaneous tissue to fully expose the carotid artery.Afterwards,the length and caliber of the blood vessel were measured.End-to-end anastomosis was performed,time was recorded and the patency of the vascular anastomosis was checked by injecting methyl violet solution into vascular cavity.SPSS 21.0 statistical software was used for analysis.Results In this group of experiments,the length of the left and right common carotid arteries was compared,and the difference was not statistically significant[the length for the left artery,(9.57±0.28)cm;the length for the right artery,(9.61±0.27)cm,t=0.779,P>0.05].The left and right rostral artery calibers were compared,and the difference was not statistically significant[the caliber for the left cranial artery,(1.12±0.08)mm;the caliber for the right cranial artery,(1.13±0.07)mm,t=0.265,P>0.05].The left and right caudal artery calibers were compared,and the difference was not statistically significant[the caliber for the left caudal artery,(0.71±0.08)mm;the caliber for the right caudal artery,(0.70±0.06)mm,t=0.063,P>0.05].The time for a single anastomosis in groups A to E was(14.36±0.60),(12.57±0.71),(10.07±0.65),(8.44±0.52),(6.27±0.74)min,and the differences were statistically significant among multiple groups(F=243.978,P<0.05).Comparison between groups A and B,groups B and C,groups C and D,and groups D and E showed statistically significant difference(t=6.053,8.209,6.178,7.543,all P<0.05).With the increase in the number of vascular anastomoses,the training effect was obvious.Conclusion The vascular anastomosis model of chicken common carotid arteries has a large vascular caliber and the model is easy to prepare.It is an effective method for learning vascular an
