摘 要:结直肠癌(CRC)血液标志物主要分为4类:核酸(RNA/DNA/信使RNA/microRNA)、细胞因子、抗体和蛋白质。在核酸中最有希望的血液标志物是NEAT;2非编码RNA, SDC2甲基化和Septin 9甲基化。检测结直肠癌的最有希望的细胞因子是白细胞介素(IL)-8, 最有希望的蛋白类标志物是CA11-19糖蛋白和DC-SIGN/DC-SIGNR蛋白。这些标志物检测原发性CRC的敏感性为70%~98%, 特异性为84%~99%。目前研究最广泛的血液标志物是Septin 9甲基化, 该标志物灵敏性范围是48%~96%。如果结合粪便免疫化学测试(FIT), 其敏感性从78%提高到94%。Septin 9甲基化或SDC2甲基化均比CEA或CA19-9具有更好的敏感性和特异性。为了系统评估血液标志物诊断结直肠癌的效能, 本文对文献报道的人类原发性结肠直肠癌的诊断性血液标志物的文献进行了系统综述。Colorectal cancer(CRC)blood markers are mainly divided into four categories:nucleic acids(RNA/DNA/messenger RNA/microRNA),cytokines,antibodies and proteins.The most promising blood marker in nucleic acids is n NEAT_v2,SDC 2 methylated DNA and Septin 9 methylated DNA.The most promising cytokine for detecting CRC is interleukin(IL)-8,and the most promising protein markers are CA11-19 glycoprotein and DC-SIGN/DC-SIGNR protein.The sensitivity of these markers in the detection of primary CRC ranges from 70%to 98%and the specificity ranges from 84%to 99%.The most intensively studied blood marker is Septin 9 methylation,with sensitivity ranging from 48%to 96%in different reports.When combined with fecal immunochemical test(FIT),the average sensitivity of CRC detection increased from 78%to 94%.Septin 9 methylation and SDC2 methylation have better sensitivity and specificity than CEA or CA19-9.In order to systematically evaluate the efficacy of blood markers in the diagnosis of CRC,this paper systematically reviews the literature on diagnostic blood markers of human primary CRC in recent years.
