超声乳化联合小梁切除术治疗老年青光眼合并白内障的临床疗效观察

摘　　要：目的探讨老年青光眼合并白内障应用超声乳化联合小梁切除术治疗的方法及临床疗效。方法90例(90眼)青光眼合并白内障老年患者,随机分为观察组和对照组,每组45例(45眼)。观察组采用超声乳化联合小梁切除术治疗,对照组采用单纯小梁切除术治疗。对比两组治疗效果、术后并发症发生情况、视力及眼压水平。结果观察组治疗总有效率为91.11%,高于对照组的68.89%,差异有统计学意义(P<0.05)。治疗后,观察组视力(0.9±0.1)高于对照组的(0.5±0.1),眼压(14.1±3.4)mm Hg(1 mm Hg=0.133 kPa)低于对照组的(15.7±3.8)mm Hg,差异均有统计学意义(P<0.05)。两组患者均未出现虹膜损伤及出血、玻璃体脱出、后囊破裂等严重并发症。观察组术后并发症发生率为8.89%,低于对照组的24.44%,差异有统计学意义(P<0.05)。结论超声乳化联合小梁切除术治疗老年青光眼合并白内障疗效显著,可有效改善患者的视力及眼压水平,并发症发生率低,对患者的身体健康以及生活质量具有十分积极的意义,值得临床推广应用。Objective To discuss the method and clinical efficacy of phacoemulsification combined with trabeculectomy in the treatment of glaucoma complicated with cataract in the elderly.Methods A total of 90 elderly patients(90 eyes)with glaucoma and cataract were randomly divided into observation group and control group,with 45 cases(45 eyes)in each group.The observation group was treated with phacoemulsification and trabeculectomy,and the control group was treated with trabeculectomy alone.The therapeutic effect,postoperative complications,visual acuity and intraocular pressure were compared between the two groups.Results The total effective rate of the observation group was 91.11%,which was higher than 68.89%of the control group,and the difference was statistically significant(P<0.05).After treatment,the visual acuity(0.9±0.1)of the observation group was higher than(0.5±0.1)of the control group,and the intraocular pressure(14.1±3.4)mm Hg(1 mm Hg=0.133 kPa)was lower than(15.7±3.8)mm Hg of the control group.All the differences were statistically significant(P<0.05).There were no serious complications such as iris injury,hemorrhage,vitreous prolapse and posterior capsule rupture in both groups.The postoperative complication rate in the observation group was 8.89%,which was lower than 24.44%in the control group,and the difference was statistically significant(P<0.05).Conclusion Trabeculectomy combined with phacoemulsification has significant efficacy in the treatment of glaucoma complicated with cataract in the elderly,and can significantly improve the visual acuity and intraocular pressure,and the complications is low.It has very positive significance for patients’health and quality of life,and is worthy of clinical promotion and application.
