甲状腺全切除术治疗甲状腺癌的临床效果及对患者甲状旁腺激素水平的改善体会

摘　　要：目的探讨甲状腺全切除术治疗甲状腺癌的临床效果及对甲状旁腺激素(PTH)水平的改善作用。方法64例甲状腺癌患者,根据随机抽签法分为观察组和对照组,各32例。对照组患者采取甲状腺次全切除术治疗,观察组采取甲状腺全切除术治疗。比较两组治疗效果、手术指标、治疗后PTH水平、并发症发生率以及治疗前后的生活质量。结果观察组治疗总有效率高于对照组,并发症发生率低于对照组,差异具有统计学意义(P<0.05)。观察组患者手术时间(77.5±10.6)min、住院时间(4.4±1.1)d均短于对照组的(123.5±16.5)min、(7.1±1.6)d,术中出血量(16.1±4.0)ml少于对照组的(35.1±5.1)ml,差异均具有统计学意义(t=13.269、7.866、16.583,P=0.000、0.000、0.000<0.05)。观察组患者治疗后1 h、1 d、3 d的PTH水平均高于对照组,差异均具有统计学意义(P<0.05)。治疗后,观察组患者功能量表、症状量表、整体健康状况量表评分分别为(78.9±6.7)、(83.3±6.3)、(70.1±6.6)分,均高于对照组的(66.8±5.0)、(74.1±4.8)、(57.1±5.3)分,差异均具有统计学意义(P<0.05)。结论甲状腺全切除术治疗甲状腺癌具有很好的效果,且并发症较少,手术操作简单,术后康复快,还有助于改善患者PTH水平和生活质量。Objective To discuss the clinical effect of total thyroidectomy in the treatment of thyroid cancer and its improvement on parathyroid hormone(PTH)level.Methods A total of 64 patients with thyroid cancer were randomly divided into observation group and control group,with 32 cases in each group.Patients in the control group were treated with subtotal thyroidectomy,and patients in the observation group were treated with total thyroidectomy.The therapeutic effect,surgical index,PTH level after treatment,complication rate,and quality of life before and after treatment were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the complication rates was lower than that in the control group.All the differences were statistically significant(P<0.05).The operation time(77.5±10.6)min and hospitalization time(4.4±1.1)d in the observation group were shorter than(123.5±16.5)min and(7.1±1.6)d in the control group,and the intraoperative blood loss(16.1±4.0)ml was less than(35.1±5.1)ml in the control group.All the differences were statistically significant(t=13.269,7.866,16.583;P=0.000,0.000,0.000<0.05).The PTH levels in the observation group at 1 h,1 d,and 3 d after treatment were higher than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the scores of functional scale,symptom scale and overall health status scale of observation group were(78.9±6.7),(83.3±6.3),and(70.1±6.6)points,which were higher than(66.8±5.0),(74.1±4.8),and(57.1±5.3)points of the control group,and the differences were statistically significant(P<0.05).Conclusion Total thyroidectomy in the treatment of thyroid cancer has a good effect,and fewer complications,simple operation,fast postoperative recovery,and also help to improve the PTH level and quality of life of patients.
