眼表综合分析仪Keratograph 5M观察翼状胬肉术前后眼表状况对比研究

摘　　要：目的对比眼表综合分析仪Keratograph 5M观察翼状胬肉患者手术前后眼表的变化。方法选取2018年6月—2019年10月惠州市中心人民医院就诊的翼状胬肉患者67例,行翼状胬肉切除联合自体角膜缘干细胞移植术,对比术前、术后眼表疾病严重程度改善情况,并采用Keratograph 5M观察术前,术后1个月,3个月,6个月患眼泪膜破裂时间、泪河高度、鼻侧结膜充血评分以及上下睑板腺红外照相后评分,对测量结果行统计学分析。结果患者OSDI评分术后1 m、3 m、6 m均低于术前,差异有统计学意义(P<0.05)。术后各时间点泪膜破裂时间均长于术前,差异有统计学意义(P<0.05),术后各时间点中央泪河高度与术前比较,差异无统计学意义(P>0.05);术后各时间点鼻侧结膜充血评分以及上下睑板腺红外照相后评分低于术前,差异有统计学意义(P<0.05)。结论翼状胬肉切除联合自体角膜缘干细胞移植术可有效降低眼表疾病严重程度,改善患者泪膜稳定性,减轻眼表炎症,提高睑板腺功能,且不影响泪液基础分泌。Objective To observe the changes of ocular surface in patients with pterygium before and after operation by keratograph 5m.Methods Patients with pterygium treated in our hospital from June 2018 to October 2019 underwent pterygium excision combined with autologous limbal stem cell transplantation.The improvement of ocular surface disease severity before and after operation was compared.Keratograph 5m was used to observe the tear film rupture time,tear river height The nasal conjunctival congestion score and the score after infrared photography of upper and lower meibomian glands were statistically analyzed.Results The osdi scores at 1m,3M and 6m after operation were lower than those before operation(P<0.05)。The tear film rupture time at each time point after operation was longer than that before operation,and the difference was statistically significant(P<0.05).The central tear river height at each time point after operation was not statistically significant compared with that before operation(P>0.05).The nasal conjunctival congestion score and the score after infrared photography of upper and lower meibomian glands at each time point after operation were lower than that before operation,and the difference was statistically significant(P<0.05).Conclusion Pterygium excision combined with autologous limbal stem cell transplantation can effectively reduce the severity of ocular surface diseases,improve the stability of tear film,reduce ocular surface inflammation,improve the function of meibomian gland,and do not affect the basic secretion of tears.
