摘 要:目的:观察Cariostat龋易感性检测技术所检测出的龋态值与3岁无龋儿童1年后龋患进展的相互关系。方法:以深圳市2所市属幼儿园就读的3岁无龋儿童为研究对象,进行口腔检查并采用Cariostat龋易感性检测技术检测牙菌斑产酸能力,追踪观察1年,计算1年后的患龋率、龋失补牙数、龋失补牙面。统计学分析不同龋态值儿童龋患状况差异。结果:2所幼儿园共261名3岁无龋儿童完成了1年的纵向观察,1年后的患龋率、龋失补牙数及龋失补牙面分别为17.62%、0.38、0.49,高龋态值儿童的患龋率、龋失补牙数、龋失补牙面均显著高于中、低龋态值儿童(P<0.05)。以3岁时无龋儿童的高龋态值为患龋风险评估指标,龋风险高、低两组无龋儿童1年后的患龋率分别为31.48%、14.01%,预测儿童1年后龋患状况的灵敏度为36.96%、特异度为82.79%。结论:Cariostat龋易感性检测方法可以筛选出龋病高危易感儿童,可有效且可靠地评估无龋儿童的患龋风险。Objective:To observe the relationship between caries status detected by Cariostat caries susceptibility test and caries progression in 3-year-old caries free children one year later.Methods:Taking 3-year-old caries free children from two municipal kindergartens in Shenzhen as the research object, oral examination was carried out, and Cariostat caries susceptibility test technique was used to detect the acid production ability of dental plaque.The oral reexamination was carried out one year later.The caries incidence, dmft and dmfs among children with different levels of Cariostat scores were statistically compared and analyzed.Results:A total of 261 3-year-old caries free children in the two kindergartens completed one-year longitudinal observation.One year later, the caries incidence, dmft and dmfs were 17.62%,0.38 and 0.49 respectively.The incidence of caries, dmft and dmfs in children with high level of Cariostat scores were significantly higher than those in children with medium and low levels of Cariostat scores(P <0.05).Taking the high level of Cariostat scores of caries free children at the age of 3 as the caries risk assessment index, the caries prevalence rates of caries free children in the high and low caries risk groups after one year were 31.48% and 14.01% respectively, and the sensitivity and specificity of predicting caries status in caries free children after one year were 36.96% and 82.79%.Conclusion:Cariostat caries susceptibility test method can screen high-risk caries susceptible children, and can effectively and reliably predict the caries risk of caries free children.
