成都地区中老年人血糖、血脂水平与肠道双歧杆菌关系探究

摘　　要：目的探索成都地区中老年人肠道双歧杆菌在不同血糖血脂水平下种群结构的特征,为双歧杆菌在高血糖、血脂异常防治中的应用提供研究基础。方法于2018年4-8月征集289名成都地区45岁及以上中老年人,分为健康组、血脂异常组、高血糖组和双高组(同时患有血脂异常及高血糖),检测其空腹血糖(GLU)、血清胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平,并收集粪便样本采用实时荧光定量PCR对其中双歧杆菌属及8种人体肠道内常见的双歧杆菌进行定量检测,分析检出情况及其与血糖血脂的相关性。结果共收集289例样本,组间双歧杆菌菌种结构分析:(1)双歧杆菌菌种的检出情况:检出率和检出种数在组间无明显差异;在数量上,角双歧杆菌在血脂异常组中高于健康组,且高于双高组,链状双歧杆菌在高血糖组中高于健康组,齿双歧杆菌在血脂异常组中高于双高组。(2)双歧杆菌数量和血糖血脂的相关性:在属水平上,仅在血脂异常组中双歧杆菌与TC呈负相关(r=-0.346)。在种水平上,健康组两歧双歧杆菌与TG呈负相关(r=-0.761),短双歧杆菌与GLU、TC、LDL-C呈负相关(r=-0.256,r=-0.261,r=-0.362),齿双歧杆菌与GLU呈正相关(r=0.206),婴儿双歧杆菌与TC呈负相关(r=-0.334)。高血糖组链状双歧杆菌、婴儿双歧杆菌均与HDL-C呈正相关(r=0.307,r=0.525);双高组两歧双歧杆菌与HDL-C呈负相关(r=-0.828),短双歧杆菌与TG、HDL-C呈正相关(r=0.427,r=0.375)。结论血脂异常、高血糖的中老年人在双歧杆菌检出情况上与健康人出现差异。相比双歧杆菌菌种的结构组成,肠道中双歧杆菌菌种数量变化与血糖、血脂水平的关联性更强,且在不同情况下表现出其独特的特征,或可成为反映糖脂代谢的指标。Objective To explore characteristics of the species structure of the genus Bifidobacterium at different levels of blood glucose and lipid in middle-aged and older adults in Chengdu so as to provide research basis for applying bifidobacteria in the prevention and treatment of hyperglycemia and dyslipidemia.Methods A total of 289 middle-aged and older adults of 45 and older were recruited in Chengdu between April and August 2018.They were divided into the healthy group,the dyslipidemia group,the hyperglycemia group,and the combination group(of subjects with both dyslipidemia and hyperglycemia).The levels of their fasting blood glucose(GLU),total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)were examined.In addition,stool samples were collected and real-time fluorescence quantitative PCR was used for quantitative analysis of the genus Bifidobacterium and the 8 bifidobacteria most commonly found in human intestines,the results of which were then examined to identify their correlation to blood glucose and lipid levels.Results A total of 289 samples were collected and findings of inter-group comparison of the species structure of Bifidobacterium were as follows:1)findings regarding the Bifidobacterium species examined--there was no significant difference between groups in the detection rate and the number of species detected;the quantity of B.angulatum was significantly higher in the dyslipidemia group than that in the healthy group and that in the combination group,the quantify of B.catenulatum was significantly higher in the hyperglycemia group than that in the healthy group,and the quantity of B.dentium was significantly higher in the dyslipidemia group than that in the combination group.2)Findings regarding the correlation between the quantity of bifidobacteria and blood glucose and lipid--at the genus level,only the dyslipidemia group showed negative correlation(r=-0.346)between Bifidobacterium and TC.At the species level,B.bifidum was n
