中医药分期治疗卒中后抑郁研究进展

摘　　要：卒中后抑郁(PSD)是指中风在诊断和治疗的期间伴发的情感障碍,可归属中医“郁证”范畴。临床上根据中风的发展可分为急性期、恢复期以及后遗症期。急性期病位责之于肝,肝气郁结多为主要病机,证多属实,治疗上以行气疏肝为法,方以逍遥散、柴胡疏肝散为主;中期主要责之于心、脾,多有痰、湿、瘀交结,证多虚实夹杂,治以化痰祛湿、理气活血,兼以健脾养心、补益气血,方选血府逐瘀汤、归脾汤等;后遗症期病涉五脏,主要责之肝、肾,多属虚证,治宜益气调中、滋补肝肾,方选益肾解郁方、健脑安神颗粒等。目前,中医药治疗PSD效果显著,现将近年来中医中药的治疗进展进行综述。Post stroke depression(PSD)refers to the affective disorder during the diagnosis and treatment of stroke.It belongs to the category of“depression syndrome”in TCM.Clinically,according to the development of stroke,it can be divided into acute stage,recovery stage and sequelae stage.In the acute stage,the main pathogenesis is liver qi stagnation,and most of the syndromes are true.In the treatment,Xiaoyao powder and Chaihu Shugan powder are the main prescriptions.In the middle stage,the main responsibilities are heart and spleen,with phlegm,dampness and blood stasis mixed together,and most of the syndromes are deficiency and excess.In the sequelae stage,the disease involves five zang organs,mainly responsible for the liver and kidney,most of which belong to deficiency syndrome.At present,the effect of traditional Chinese medicine in the treatment of PSD is remarkable.The progress of traditional Chinese medicine in recent years is summarized as follows.
