内界膜剥除、翻转填塞辅助治疗高度近视黄斑裂孔性视网膜脱离患者的疗效评价

摘　　要：目的评价内界膜剥除、翻转填塞治疗高度近视黄斑裂孔性视网膜脱离(MHRD)患者的疗效。方法选取我院2018年1月~2019年3月高度近视MHRD患者28例,均行内界膜剥除、翻转填塞辅助25G微创玻璃体切割手术治疗,对比术前及术后1个月、3个月、6个月BCVA、黄斑区视网膜厚度,并统计黄斑区裂孔闭合率及视网膜复位率。结果术后1个月黄斑裂孔闭合率及视网膜复位率为85.7%(24/28);术后6个月logMAR BCVA优于术前、术后1个月、3个月(F=40.311,P<0.001);术后6个月黄斑区视网膜厚度小于术前、术后1个月、3个月(F_(鼻纤维)=30.160;F_(鼻细胞)=75.036;F_(鼻丛)=47.138;F_(鼻核)=21.254;F_(颞纤维)=13.512;F_(颞细胞)=262.732;F_(颞丛)=163.615;F_(颞核)=35.445,P<0.05)。结论内界膜剥除、翻转填塞辅助25G微创玻璃体切割手术治疗高度近视MHRD效果确切,可提高患者视力,改善黄斑区视网膜厚度。Objetive To evaluate the curative effect of internal limiting membrane peeling and turnover packing in the treatment of patients with high myopia macular hole retinal detachment(MHRD).Methods A total of 28 MHRD patients with high myopia from January 2018 to March 2019 in our hospital were selected.All underwent surgical treatment of internal limiting membrane stripping,turnover packing assisted 25G minimally invasive vitrectomy.The BCVA and retinal thickness in the macular area were compared before and 1 month,3 months,and 6 months after the operation,and the rate of hole closure in the macular area and retinal reattachment rate were calculated.Results The macular hole closure rate and retinal reattachment rate were 85.7%(24/28)one month after surgery;LogMAR BCVA at 6 months after operation was better than before operation,1 month and 3 months after operation(F=40.311,P<0.001);The retinal thickness in the macular area at 6 months after surgery was less than that before surgery,1 month and 3 months after surgery(F_(nasal fiber)=30.160;F_(nasal cell)=75.036;F_(nasal plecus)=47.138;F_(nasal nucleus)=21.254;F_(temoral fiber)=13.512;F_(temporal cell)=262.732;F_(temoral plexus)=163.615;F_(temporal nucles)=35.445,P<0.05).Conclusion Removal of the inner limiting membrane,turnover and packing assisted by 25G minimally invasive vitrectomy surgery to treat high myopia MHRD has a definite effect,which can improve the vision of patients and improve the retinal thickness in the macular area.
