四君子汤加减联合常规西药治疗慢性萎缩性胃炎脾胃虚弱证临床研究

摘　　要：目的:观察四君子汤加减联合常规西药治疗慢性萎缩性胃炎脾胃虚弱证的临床疗效。方法:选取120例慢性萎缩性胃炎脾胃虚弱证患者,按随机数字表法分为观察组和对照组各60例。对照组给予常规西药治疗,观察组在对照组基础上给予四君子汤加减治疗。比较2组临床疗效、中医证候评分、胃黏膜病理情况及炎症因子水平。结果:观察组临床总有效率优于对照组(χ_(2)=5.551,P=0.018)。治疗后,2组各项中医症状评分均低于治疗前(P<0.05);观察组治疗后各项中医症状评分低于对照组(P<0.05)。治疗后,2组胃腺体萎缩、肠上皮化生程度较治疗前明显减轻(P<0.05),观察组治疗后胃腺体萎缩、肠上皮化生程度轻于对照组(P<0.05)。治疗后,2组血清人表皮生长因子(EGF)、白细胞介素-32(IL-32)水平均低于治疗前,降钙素基因相关肽(CGRP)水平高于治疗前(P<0.05);观察组治疗后EGF、IL-32水平低于对照组,CGRP水平高于对照组(P<0.05)。结论:四君子汤加减联合常规西药治疗慢性萎缩性胃炎脾胃虚弱证疗效确切,有利于减轻患者临床症状及胃黏膜炎性损伤,改善胃黏膜组织。Objective:To observe the clinical effect of the therapy of modified Sijunzi tang combined with conventional western medicine on the treatment of chronic atrophic gastritis with spleen-stomach weakness syndrome.Methods:A total of 120 patients with chronic atrophic gastritis with spleen-stomach weakness syndrome were selected and divided into the observation group and the control group according to random number table method,with 60 cases in each group.The control group was treated with conventional western medicine,and the observation group was additionally treated with modified Sijunzi tang based on the treatment of the control group.The clinical effects,Chinese medicine syndrome scores,gastric mucosal pathology and levels of inflammatory factors were compared between the two groups.Results:The total effective rate in the observation group was better than that in the control group(χ_(2)=5.551,P=0.018).After treatment,scores of Chinese medicine symptoms in both groups were decreased when compared with those before treatment(P<0.05);the scores of Chinese medicine symptoms in the observation group were lower than those in the control group(P<0.05).After treatment,the degree of gastric gland atrophy and intestinal metaplasia in the two groups was significantly relieved when compared with that before treatment(P<0.05),and the above degree in the observation group was less than that in the control group(P<0.05).After treatment,the levels of human epidermal growth factor(EGF)and interleukin-32(IL-32)in serum in both groups were decreased when compared with those before treatment,and the levels of calcitonin gene-related peptide(CGRP)were increased(P<0.05).After treatment,the levels of EGF and IL-32 in the observation group were lower than those in the control group,and CGRP level was higher(P<0.05).Conclusion:The therapy of modified Sijunzi tang combined with conventional western medicine is effective in the treatment of chronic atrophic gastritis with spleen-stomach weakness syndrome,which is beneficial to alleviating
