针刺联合康复锻炼治疗脑卒中后肩手综合征临床研究

摘　　要：目的:观察针刺联合康复锻炼对脑卒中后肩手综合征患者上肢功能、疼痛程度及肩手综合征评价量表(SHSS)评分的影响。方法:选取脑卒中后肩手综合征患者77例,按随机数字表法分为对照组38例与针刺组39例。2组均采用药物常规治疗,同时对照组进行康复锻炼,针刺组在康复锻炼同时加用针刺治疗。比较2组临床疗效及治疗前后上肢功能、疼痛程度及SHSS评分。结果:针刺组总有效率94.87%,高于对照组81.58%,但差异无统计学意义(P>0.05)。与同组治疗前,治疗后2组上肢简化Fugl-Meyer运动评分量表(FMA)评分升高(P<0.05),疼痛视觉模拟评分法(VAS)评分、SHSS评分降低(P<0.05);与对照组治疗后比较,针刺组治疗后FMA评分较高(P<0.05),VAS、SHSS评分较低(P<0.05)。结论:针刺联合康复锻炼可缓解脑卒中后肩手综合征患者临床症状,减轻疼痛程度,改善上肢功能。Objective:To observe the effect of acupuncture combined with rehabilitation exercise on upper limb function,pain degree and scores of Shoulder-Hand Syndrome Scale(SHSS)of patients with shoulder-hand syndrome after stroke.Methods:A total of 77 cases of patients with shoulder-hand syndrome after stroke were selected and divided into the control group and the acupuncture group according to the random number table method,with 38 and 39 cases in each group respectively.Both groups were treated with routine medication therapy;the control group was additionally given rehabilitation exercise,and the acupuncture group was additionally treated with acupuncture combined with rehabilitation exercise.Upper limbs function,pain degree and SHSS scores before and after treatment as well as clinical effects were compared between the two groups.Results:The total effective rate was 94.87%in the acupuncture group,higher than that of 81.58%in the control group,but there was no significance in the difference(P>0.05).When compared with those in the same group before treatment,scores of simplified Fugl-Meyer Assessment(FMA)for upper limb in the two groups were increased(P<0.05),scores of Visual Analogue Scale(VAS)for pain and SHSS were decreased(P<0.05).After treatment,FMA score in the acupuncture group was higher than that in the control group(P<0.05),and scores of VAS and SHSS were lower(P<0.05).Conclusion:Acupuncture combined with rehabilitation exercise can relieve clinical symptoms and pain degree of patients with shoulder-hand syndrome after stroke,and improve their upper limb function.
