准分子激光角膜上皮瓣下磨镶术治疗近视患者的效果

摘　　要：目的:观察准分子激光角膜上皮瓣下磨镶术治疗近视患者的效果。方法:回顾性分析2018年4月至2020年6月该院收治的145例近视患者的临床资料。根据手术方案不同分为观察组(n=74,148眼)与对照组(n=71,共142眼),对照组行准分子激光屈光性角膜切削术治疗,观察组行准分子激光角膜上皮瓣下磨镶术治疗,比较两组上皮愈合时间、疼痛程度[视觉模拟评分法(VAS)]评分、Haze评分、屈光度数、裸眼视力和术后并发症发生率。结果:观察组角膜上皮愈合时间长于对照组,差异有统计学意义(P<0.05);术后1、3、7、15 d,观察组VAS评分均低于对照组,差异有统计学意义(P<0.05);术后1、3、6个月,观察组Haze评分均低于对照组,差异有统计学意义(P<0.05);术后1、3、6个月,观察组屈光度数、裸眼视力与对照组比较,差异均无统计学意义(P>0.05);观察组并发症发生率为3.38%,与对照组的4.93%比较,差异无统计学意义(P>0.05)。结论:准分子激光角膜上皮瓣下磨镶术治疗近视患者降低术后疼痛评分和Haze评分的效果优于准分子激光屈光性角膜切削术,但角膜上皮愈合时间延长。Objective:To observe effects of excimer laser subepithelial keratomileusis in treatment of myopia patients.Methods:The clinical data of 145 myopia patients admitted to our hospital from April 2018 to June 2020 were retrospectively analyzed.According to different surgical plans,they were divided into observation group(n=74,148 eyes)and control group(n=71,142 eyes in total).The control group was treated with excimer laser photorefractive Keratectomy,while the observation group was treated with excimer laser subepithelial keratomileusis.The epithelial healing time,the pain level[visual analogue scale(VAS)]score,the Haze score,the diopter,the naked visual acuity and the incidence of postoperative complications were compared between the two groups.Results:The corneal epithelial healing time in the observation group was longer than that in the control group,and the difference was statistically significant(P<0.05).1,3,7,and 15 days after the surgery,the VAS scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).1,3,and 6 months after the surgery,the Haze score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).1,3,and 6 months after the surgery,there were no significant differences in diopter and naked visual acuity between the observation group and the control group(P>0.05).Further,the incidence of complications in the observation group was 3.38%,compared with 4.93% in the control group,the difference was not statistically significant(P>0.05).Conclusions:Excimer laser subepithelial keratomileusis can reduce the postoperative pain score and the Haze score in the patients with myopia.Its effect is better than that of excimer laser photorefractive Keratectomy.However,the corneal epithelial healing time is prolonged.
