典必殊眼膏治疗睑板腺功能障碍相关干眼临床疗效观察

摘　　要：目的 观察典必殊眼膏外用联合物理治疗睑板腺功能障碍干眼症的临床疗效。方法 将宜川县人民医院眼科门诊2017年01月至2019年12月诊断为睑板腺功能障碍相关干眼症患者45例,采用随机数字表法分为三组:A组、B组及C组;A组:给予妥布霉素地塞米松眼膏物理治疗+玻璃酸钠滴眼液4次/d。B组:给予普拉洛芬滴眼液+玻璃酸钠滴眼液4次/d+物理治疗。C组:给予玻璃酸钠滴眼液4次/d+物理治疗。分别观察各组在治疗14 d、28 d后,泪河高度、泪膜破裂时间(tear break-up time,TBUT)、基础泪液分泌试验(schirmer I test,SIT)和临床症状。结果 治疗14 d后各组患者在泪河高度、TBUT、SIT及临床症状上均有明显改善(P<0.05)。随着治疗时间的延长,治疗28 d后各组间在泪河高度、TBUT、SIT和临床症状改善方面差异均具有统计学意义。且随着治疗时间延长各组内在泪河高度、TBUT、SIT和临床症状改善方面差异具有统计学意义(P<0.05)。结论 妥布霉素地塞米松眼膏+玻璃酸钠滴眼液联合物理治疗能有效地改善睑板腺功能障碍干眼症患者的泪河高度、TBUT、SIT及临床症状,改善睑板腺功能。Objective To explore the clinical effect of tobradex ointment in the treatment of dry eye caused by meibomian gland dysfunction.Methods 45 cases of dry eye associated with meibomian gland dysfunction in Yichuan County People’s Hospital from January 2017 to December 2019 were randomly divided into three groups:group A,B and C.Group A:after physical therapy with tobramycin and dexamethasone eye ointment,external use+sodium hyaluronate eye drops,4 times a day.Group B:pranoprofen eye drops+sodium hyaluronate eye drops,4 times a day+Physical therapy.Group C:sodium hyaluronate eye drops,4 times a day+physical therapy.The tear meniscus height,tear break-up time(TBUT),schirmer I test(SIT) and clinical symptoms of each group were observed at 14 d and 28 d after treatment.Results After 14 days of treatment,the tear meniscus height,TBUT,SIT and clinical symptoms of each group were significantly improved(P<0.05).With the prolongation of treatment time,there were statistically significant differences in tear meniscus height,TBUT,SIt and clinical symptoms improvement among the groups after 28 days of treatment.And with the prolongation of treatment time,there were significant differences in tear meniscus height,TBUT,SIt and clinical symptoms improvement in each group(P<0.05).Conclusion The tobramycin and dexamethasone eye ointment+sodium hyaluronate eye drops combined with physical therapy can effectively improve the clinical symptoms of patients with meibomian gland dysfunction xerophthalmia,and it can also improve tarsal gland function.
