耐高压静脉套管针对乳腺癌患者化疗后MRI检查中对比剂渗漏的影响

摘　　要：目的探讨耐高压静脉套管针对乳腺癌患者化疗后MRI检查中对比剂渗漏的影响。方法根据入组顺序单双号将114例乳腺癌患者分为对照组(n=57)和观察组(n=57),对照组采用带翼头皮针,观察组采用耐高压静脉套管针。比较两组患者的穿刺情况、图像质量、负性情绪[焦虑自评量表(SAS)和抑郁自评量表(SDS)]、检查配合度、满意度。结果观察组患者对比剂外渗、针头堵塞、血液逆流发生率均低于对照组(P﹤0.05),一次性穿刺成功率高于对照组(P﹤0.05)。观察组患者的图像质量评分明显高于对照组患者(P﹤0.01)。检查后,两组患者SAS、SDS评分均低于本组检查前(P﹤0.05),且观察组患者SAS、SDS评分均低于对照组(P﹤0.05)。观察组患者的检查配合度为94.74%,高于对照组患者的78.95%(P﹤0.05),满意度为96.49%,高于对照组患者的82.46%(P﹤0.05)。结论耐高压静脉套管针能够提高乳腺癌化疗后行MRI的图像质量,减少对比剂的渗漏和不良反应,减轻负性情绪,提高检查配合度和满意度。Objective To investigate the effect of hyperbaric venous trocar on contrast agent leakage in MRI examinations of breast cancer patients after chemotherapy.Method A total of 114 patients with breast cancer were divided into the control group(n=57)and observation group(n=57)according to the odd and even numbers in the order of enrollment.The control group was given winged scalp needle,and the observation group was given hyperbaric venous trocar.The occurrence of the puncture process,image quality,negative emotions[self-rating anxiety scale(SAS),and self-rating depression scale(SDS)],inspection cooperation,and satisfaction were compared between the two groups of patients.RRee-sult The incidences of contrast medium extravasation,needle blockage and blood reflux in the observation group were lower than those in the control group(P<0.05),and the success rate of one-time puncture was higher than that in the control group(P<0.05).The image quality score of the observation group was significantly higher than that of the control group(P<0.01).After the examination,the SAS and SDS scores of the two groups were lower than those before the examination(P<0.05),and the SAS and SDS scores of the observation group were lower than those of the control group(P<0.05).The inspection cooperation degree of patients in the observation group was 94.74%,which was higher than that of the patients in the control group 78.95%(P<0.05),and the satisfaction rate was 96.49%,which was higher than that of the patients in the control group 82.46%(P<0.05).Conclusion The employment of the hyperbaric venous trocar could improve the image quality of MRI,reduce the leakage of contrast agent,risk of adverse events,and patients’negative emotions,and improve their cooperation and satisfaction for the inspection.
