飞秒激光辅助超声乳化术与传统超声乳化术的临床效果比较

摘　　要：目的比较飞秒激光辅助超声乳化手术与传统超声乳化手术的临床效果。方法前瞻性随机对照研究。将2019年1月至2020年12月郑州爱尔眼科医院收治的白内障42例(78眼)随机分组为飞秒激光辅助手术组20例(37眼)和常规手术组22例(41眼), 两组均植入多焦点人工晶状体。术后随访3个月比较两组效果。结果飞秒组的术后裸眼远、近视力及残余屈光度(等效球镜度)分别为(0.07±0.05)D、(0.14±0.07)D及(-0.04±0.33)D;超乳组分别为(0.09±0.06)D、(0.15±0.09)D及(-0.05±0.29)D。两组患者差异均无统计学意义(t=0.03, 0.06及0.10;P=0.512, 0.522及0.540)。全眼像差(总高阶像差、彗差、三叶草像差和球差)差异均无统计学意义(均P>0.05)。眼内总高阶像差飞秒组低于常规组(t=1.68, P=0.049)。飞秒组患者满意度评分高于常规组(Z=-1.88, P=0.034)。戴镜时间和光干扰现象两组间差异无统计学意义。结论飞秒激光辅助超乳手术和常规超乳手术均植入AcrySof ReSTOR +3D IOL术后患者的屈光状态和视力均无差异, 然而飞秒激光组术后眼内的高阶像差低于常规组, 满意度高于常规组。Objective To compare the clinical efficacy between femtosecond laser-assisted phacoemulsification and conventional phacoemulsification.Methods This was a prospective random control study.A total of 78 eyes of 42 patients were randomly divided into 37 eyes of 20 cases as the femtosecond laser assisted phacoemulsification group and 41 eyes of 22 cases as the conventional phacoemulsification group.Multifocal intraocular lens was implanted in both groups.The follow-up time was 3 months after operation.The efficacy between the two groups were compared.Correlations between target-induced astigmatism(TIA)and surgically induced astigmatism(SIA)were also compared between the groups.Results The far and near uncorrected visual acuity and residual spherical equivalent were 0.07±0.05,0.14±0.07 and(-0.04±0.33)D respectively in the femtosecond group;0.09±0.06,0.15±0.09 and(-0.05±0.29)D in the conventional phacoemulsification group,respectively.There was no significant difference between the two groups(t=0.03,0.06 and 0.10;P=0.512,0.522 and 0.540).The ocular aberrations including total higher-order aberration,coma aberration,trefoil aberration and spherical aberration between the two groups were not statistically significant(all P>0.05).Internal high-order aberrations was significantly smaller in the femtosecond group than that in the conventional phacoemulsification group(t=1.68,P=0.049).Satisfaction scores in the femtosecond group were significantly higher than that in conventional group(Z=-1.88,P=0.034).There were no significant difference between the two groups in spectacles dependence and optical interference phenomenon.Conclusion The difference of the refractive status and visual acuity between femtosecond laser phacoemulsification and the conventional phacoemulsification for the AcrySof ReSTOR 3.00D add IOL are not statistically significant.However,the internal higher-order aberrations in the femtosecond group are lower and the satisfaction are higher than that in the conventional group.
