探讨老年进展期胃癌患者行腹腔镜辅助D2式胃癌根治术的临床疗效

摘　　要：目的探讨老年进展期胃癌患者行腹腔镜辅助D2式胃癌根治术的临床疗效和安全性。方法70例老年进展期胃癌患者,随机分为观察组与参照组,各35例。参照组行开腹手术,观察组行腹腔镜辅助D2式胃癌根治术。比较两组术后恢复时间、并发症发生情况。结果观察组的肠鸣音恢复时间(23.21±4.75)h、肛门排气恢复时间(48.02±6.11)h、住院时间(8.11±1.95)d均短于参照组的(29.15±5.90)h、(57.48±7.50)h、(10.61±2.52)d,差异有统计学意义(P<0.05)。观察组并发症发生率8.57%低于参照组的31.43%,差异有统计学意义(P<0.05)。结论对老年进展期胃癌患者实行腹腔镜辅助D2式胃癌根治术治疗,患者术后恢复时间明显缩短,并发症减少,安全性和有效性理想,具有临床推广价值。Objective To discuss the clinical efficacy and safety of laparoscopic assisted D2 radical gastrectomy in elderly patients with advanced gastric cancer.Methods A total of 70 elderly patients with advanced gastric cancer were randomly divided into observation group and reference group,with 35 cases in each group.The control group underwent laparotomy,and the observation group underwent laparoscopic assisted D2 radical gastrectomy.The postoperative recovery time and complications were compared between the two groups.Results The bowel sounds recovery time(23.21±4.75)h,anal exhaust recovery time(48.02±6.11)h,and hospitalization time(8.11±1.95)d in the observation group were all shorter than those of(29.15±5.90)h,(57.48±7.50)h,and(10.61±2.52)d in the control group,and the difference was statistically significant(P<0.05).The complication rates in the observation group was 8.57%,which was lower than 31.43% in the reference group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic assisted D2 radical gastrectomy for elderly patients with advanced gastric cancer can shorten postoperative recovery time,reduce complications,and has ideal safety and effectiveness,which contains the value of clinical promotion.
