基于磁共振血管成像的西藏高原地区正常汉族人Willis环完整性及形态观察

摘　　要：目的基于磁共振血管成像(MRA)观察西藏高原地区正常汉族人群的Willis环完整性及形态,探讨高原低氧环境对正常汉族人群Willis环的影响。方法接受MRI检查的居住于西藏高原地区的正常汉族人群201例,均接受头颅MRI常规扫描+3D-TOF MRA检查,在MRA图像上观察其Willis环是否完整及Willis环形态(包括前循环形态、后循环形态),计算Willis环完整率,并观察不同高原居住时间、性别、饮酒史、吸烟史等对Willis环完整率的影响。结果201例西藏高原地区正常汉族人群中完整型Willis环42例,Willis环完整率为20.9%。高原居住时间10年以上参与者的Willis环完整率与高原居住时间10年以下参与者相比,男性参与者的Willis环完整率与女性参与者相比,有饮酒史参与者的Willis环完整率与无饮酒史参与者相比,有吸烟史参与者的Willis环完整率与无吸烟史参与者相比,P均>0.05。共发现5种不同的Willis环前循环形态,其中标准型162例、两个前交通动脉(AcomA)型4例、单侧大脑前动脉A1段发育不良型1例、单侧大脑前动脉A1段缺失型12例、AcomA缺失型22例。共发现16种不同的Willis环后循环形态,其中成人型37例、过渡型2例、单侧胚胎型7例、双侧胚胎型2例、双侧后交通动脉(PcomA)缺失型36例、双侧PcomA发育不良型22例、单侧PcomA缺失型49例、单侧PcomA发育不良型12例、完全胚胎型17例、一侧PcomA及P1发育不良对侧PcomA缺失型3例、一侧PcomA缺失对侧P1段发育不良型3例、一侧PcomA及对侧P1段缺失型1例、双侧P1段缺失型1例、一侧P1段缺失对侧发育不良型5例、一侧PcomA发育不全对侧P1缺失型3例、一侧PcomA对侧P1段发育不良型1例。结论201例西藏高原地区正常汉族人群的Willis环完整率较低,不同高原居住时间、性别、饮酒史、吸烟史等对Willis环的完整率无影响;201例西藏高原地区正常汉族人群的Willis环形态存在广泛变异,以后�Objective To observe the integrity and morphology of Willis ring in the normal Han population in Tibetan p lateau area based on magnetic resonance angiography(MRA) and to investigate the effect of plateau hyp oxic environment on Willis rings in normal Han population.Methods A total of 201 normal Han people living in the Tibet plateau who received MRI examination underwent routine MRI scanning+3 D-TOF MRA examination.The Willis ring integrity and Willis ring morphology(including anterior circulation morphology and posterior circulation morphology) were observed on MRA images,and the Willis ring integrity rate was calculated.The effects of different plateau residence time,gender,drinking history and smoking history on the Willis circle integrity rate were observed.Results Forty-two cases had complete Willis ring in 201 normal Han population in Tibet plateau area,and the complete rate of Willis ring was 20.9%.No significant differences were found in the integrity rates of the Willis ring between participants who lived at high altitude for more than 10 years and participants who lived at high altitude for less than 10 years,between male participants and female participants,between participants with drinking history and participants without drinking history,and between participants with smoking history and participants without smoking history(all P>0.05).Five types of anterior circulation of Willis ring were found,including 162 cases of normal anterior circle,4 cases of two anterior communicating arteries(AcomA),1 case of A1 hypoplasia of the anterior cerebral artery(ACA),12 cases of A1 of the ACA absent and 22 cases of AcomA absent.Sixteen types of posterior circulation of Willis ring were found,including 37 cases of adult-type posterior circle,2 cases of transitional type,7 cases of unilateral fetal-type posterior circle(FTP),2 cases of bilateral FTP,36 cases of absence of the bilateral posterior communicating arteries(PcomA),22 cases of bilateral PcomA hypoplasia,49 cases of unilateral PcomA deletion,12 cases of un
