不同固定时间对脑胶质瘤免疫组化染色质量的影响

摘　　要：目的研究免疫标记物胶质纤维酸性蛋白(glial fibrillary acidic protein,GFAP)、增殖指数(proliferation index,Ki67)在脑胶质瘤标本中因固定时间不同而对免疫组化染色结果产生的影响,以及GFAP和Ki67阳性表达的差异。方法回顾性收集2018年1月至2019年12月上海市静安区中心医院71例脑胶质瘤标本,肿瘤直径≥2cm,每例标本取6块大小为1 cm×1 cm×0.2 cm的肿瘤组织。4%甲醛固定处理,固定4 h、24 h、48 h、96 h、7 d和14d后进行GFAP、Ki67免疫组化染色,比较不同固定时间对GFAP、Ki67阳性率的影响。结果脑胶质瘤标本经过4 h、24 h、48 h、96 h、7 d和14 d固定处理后,GFAP有效阳性率分别为63.38%、97.18%、95.77%、92.95%、90.14%和77.46%,差异有统计学意义(χ^(2)=51.802,P<0.001),Ki67有效阳性率分别为87.32%、100%、100%、100%、98.59%和90.14%,差异有统计学意义(χ^(2)=30.45,P<0.001)。固定24 h、48 h、96 h和7 d的组别中GFAP、Ki67的有效阳性率比较,组间差异无统计学意义。结论不同固定时间对脑胶质瘤组织中GFAP、Ki67的免疫组化染色结果有影响,固定24 h、48 h、96 h、7 d的染色结果优于固定4 h和14 d。Objective To study the effects of immunological markers glial fibrillary acidic protein(GFAP)and proliferation index(Ki67)on the results of immunohistochemical staining in glioma specimens due to different fixation times,and to explore the difference in the positive expression of GFAP and Ki67.The difference in the positive expression of GFAP and Ki67.Methods Seventy-one cases of brain glioma specimens with tumor diameter≥2 cm were collected during Jan 2018 to Dec 2019 in Jing’an District Central Hospital of Shanghai.Six tumor tissues of 1 cm×1 cm×0.2 cm were taken from each specimen.They were fixed in 4%formaldehyde for 4 hours,24 hours,48 hours,96 hours,7 days and 14days.Immunohistochemical staining of GFAP and Ki67 was performed to compare the effects of different fixation time on the valid positive rates of GFAP and Ki67.Results After 4-hour,24-hour,48-hour,96-hour,7-day and 14-day fixation of glioma specimens,the valid positive rates of GFAP were 63.38%,97.18%,95.77%,92.95%,90.14%and 77.46%with the statistically significant difference(χ^(2)=51.802,P<0.001),and the valid positive rates of Ki67 were 87.32%,100%,100%,100%,98.59%and 90.14%with the statistically significant difference(χ^(2)=30.45,P<0.001).There was no significant difference in the valid positive rates of GFAP and Ki67 in the groups fixed for 24 hours,48 hours,96 hours and 7 days.Conclusion Fixation time had influence on the immunohistochemical staining results of GFAP and Ki67 in brain glioma specimens,and the staining results of fixation for 24 hours,48 hours,96 hours and 7 days were better than those for 4 hours and 14 days.
