摘 要:目的探讨早期检测血清S100β蛋白及神经元特异性烯醇化酶(NSE)对急性一氧化碳中毒迟发性脑病(DEACMP)的预测价值。方法选取2018年11月至2021年3月就诊于郑州市第二人民医院的69例急性一氧化碳中毒(ACMP)患者,根据入院后60d内是否发生DEACMP将患者分为DEACMP组和非DEACMP组。所有受试者在一氧化碳中毒后24h内入院并进行血清S100β蛋白、NSE的检测和分析。结果27例ACMP患者纳入DEACMP组,42例ACMP患者纳入非DEACMP组;DEACMP组患者血清S100β蛋白及NSE水平高于非DEACMP组(P<0.001);S100β及NSE水平升高是ACMP患者发生DEACMP的独立危险因素(P<0.05);S100β及NSE水平升高预测DEACMP的灵敏性分别为0.741和0.667,特异性别为0.836和0.810,两指标联合检测对ACMP患者进展为DEACMP的预测价值的曲线下面积为0.906,灵敏性提升至0.778,特异性提升至0.857。结论ACMP患者发病早期血清S100β蛋白及NSE水平升高与DEACMP的发生有关,早期监测血清S100β蛋白及NSE水平可对DEACMP的预防、病情判断和早期干预提供依据。Objective To investigate the predictive value of serum S100βprotein and neuron specific enolase(NSE)in the incidence of delayed encephalopathy after acute carbon monoxide poisoning(DEACMP).Methods A total of 69 patients with acute carbon monoxide poisoning(ACMP)who were treated in Zhengzhou Second Hospital from November 2018 to March 2021 were selected.According to whether DEACMP occurred within 60 days after admission,the patients were divided into DEACMP group and non-DEACMP group.Serum levels of S100βprotein and NSE were measured within 24 hours after carbon monoxide poisoning.Results A total of 27 patients were enrolled in DEACMP group and 42 patients in non-DEACMP group.The levels of serum S100p and NSE in DEACMP group were higher than those in non-DEACMP group(P<0.001).And the increase of serum S100p and NSE levels were independent risk factors for ACMP patients to progress to DEACMP(P<0.05).The sensitivity of S100βand NSE for predicting DEACMP were 0.741 and 0.667,respectively.The specificity of S100βand NSE for predicting DEACMP were 0.836 and 0.810,respectively.The area under the curve of the predictive value of the combined detection of the two indexes for the progression of ACMP patients to DEACMP increased to 0.906,the sensitivity increased to 0.778 and the specificity increased to 0.857.Conclusion The elevation of serum S100βprotein and NSE levels in the early stage of ACMP patients is related to the occurrence of DEACMP.Early monitoring of serum S100p protein and NSE levels can provide a basis for the prevention,disease judgment and early intervention of DEACMP.
