全瓷冠修复过程中颈缘位置设定对牙周组织保护的影响

摘　　要：目的探究全瓷冠修复过程中颈缘位置设定对牙周组织保护的影响。方法选取2017年5月至2019年5月三明市第二医院口腔科160例全瓷冠修复治疗患者为研究对象,按颈缘位置不同分为对照组(颈缘位于龈下患者)80例、观察组(颈缘位于龈上患者)80例,并在全瓷牙修复过程中注意保护牙周组织。比较两组牙石指数(CI)、菌斑指数(PLI)、牙龈指数(GI)及裂纹、牙龈缘黑线以及牙龈炎的不良反应发生率。结果修复前,两组CI、PLI、GI比较,差异无统计学意义(P>0.05);修复后两组CI、PLI、GI均有所上升,观察组CI、PLI、GI低于对照组,差异有统计学意义(P<0.05);观察组裂纹、牙龈缘黑线以及牙龈炎的不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论对该疾病患者实施全瓷冠修复过程中颈缘位置设定在龈上对牙周组织保护有良好作用,可有效起到保护作用,但对龈下患者有明显不利影响,容易造成牙周组织损伤。Objective To explore the influence of position setting of the cervical margin on the protection of periodontal tissue during the all-ceramic crown restoration.Methods A total of 160 patients with all-ceramic crown restoration in the Department of Stomatology of Sanming Second Hospital from May 2017 to May 2019 were selected as the research subjects.According to the position of the cervical margin,they were divided into 80 cases in the control group(patients with cervical margin of the restoration under the gingiva)and 80 cases in the observation group(patients with cervical margin of the restoration above the gingiva).The periodontal tissue was protected during the process of porcelain tooth restoration.The calculus index(CI),plaque index(PLI),gingival index(GI),and the incidence of adverse reactions of cracks,black lines on the gingival margin,and gingivitis were compared between the two groups.Results Before restoration,there were no statistically significant differences in CI,PLI,and GI between the two groups(P>0.05);after restoration,the CI,PLI,and GI of the two groups increased.The CI,PLI,and GI of the restoration of the observation group were lower than those of the control group,with statistically significant differences(P<0.05);the total incidences of adverse reactions of cracks,black lines on the gingival margin and gingivitis in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).Conclusion Setting the position of the cervical margin on the gingival during the restoration of the all-ceramic crown in the patient with this disease has a good effect on the protection of the periodontal tissue.However,it has obvious adverse effects on patients with cervical margin under the gingiva,and it is easy to cause periodontal tissue damage.
