摘　　要：目的:探讨固定-活动义齿联合修复对牙列缺损患者咀嚼功能、OHIP-14评分及龈沟液炎症因子表达的作用。方法:选取2016年8月-2018年4月笔者医院收治的112例牙列缺损患者进行前瞻性随机对照研究,以随机数字表法分为研究组(n=56)和对照组(n=56)。对照组给予活动义齿修复,研究组给予固定-活动义齿联合修复。比较两组治疗前、治疗后1个月、3个月咀嚼效率、龈沟液炎症因子指标[肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)]水平,并随访2年,统计两组疗效与治疗前、治疗后1年及2年口腔健康影响程度量表14(OHIP-14)评分、牙周指数[牙龈指数(GI)、龈沟出血指数(SBI)]水平、美观满意度及舒适度评分。结果:治疗后2年,研究组总有效率93.36%,高于对照组的83.64%,差异有统计学意义(P<0.05);治疗后1个月、3个月,两组咀嚼效率较治疗前提高,且研究组高于对照组,差异有统计学意义(P<0.05);两组治疗后1个月、3个月龈沟液TNF-α、hs-CRP水平较治疗前提高,但研究组低于对照组(P<0.05);治疗后1年、2年OHIP-14评分、GI、SBI水平较治疗前降低,且研究组低于对照组,差异有统计学意义(P<0.05);两组治疗后1年、2年美观满意度和舒适度评分较治疗前提高,且研究组高于对照组,差异有统计学意义(P<0.05)。结论:固定-活动义齿联合修复应用于牙列缺损患者效果显著,能有效改善咀嚼效率,减轻炎症反应,降低牙周指数,提高患者生活质量、美观满意度及舒适度。Objective To investigate the effect of combined fixed-removable denture restoration on masticatory function,OHIP-14 score and expression of inflammatory factors in gingival crevicular fluid in patients with dentition defect.Methods From August 2016 to April 2018,112 patients with dentition defect in our hospital were selected for a prospective randomized controlled study.They were divided into study group(n=56)and control group(n=56)by random number table.The repair of movable dentures was given in the control group,and the combined repair of fixed-removable dentures was given in the study group.The masticatory efficiency and inflammatory factor indexes of gingival crevicular fluid[tumor necrosis factor-α(TNF-α),high-sensitivity C-reactive protein(hs-CRP)]levels were compared between the two groups before treatment,1 month and 3 months after treatment.They were followed up for 2 years,and the curative effects,the oral health impact profile(OHIP-14)scores,periodontal index[gingival index(GI),sulcular bleeding index(SBI)]levels,degree of aesthetic satisfaction,and degree of comfort scores before treatment,1 year and 2 years after treatment of the two groups were counted.Results The total effective rate of the study group at 2 years after treatment was 93.36%higher than 83.64%in the control group(P<0.05).One month and three months after treatment,the masticatory efficiency was higher than before treatment,and the study group was higher than the control group(P<0.05).The levels of TNF-αand hs-CRP in gingival crevicular fluid were higher than before treatment in the two groups at 1 month and 3 months after treatment,but the study group was lower than the control group(P<0.05).The OHIP-14 score,GI and SBI levels at 1 and 2 years after treatment were lower than before treatment,and the study group was lower than the control group(P<0.05).The aesthetic satisfaction and comfort scores of the two groups at 1 and 2 years after treatment were higher than those before treatment,and the study group was higher than the contr
