自拟豁痰开窍方联合针灸治疗脑梗死的临床疗效观察

摘　　要：目的探讨脑梗死采用自拟豁痰开窍方联合针灸治疗的临床疗效。方法选取84例脑梗死患者,按随机数字表法分为研究组和对照组各42例,对照组接受常规治疗,研究组在此基础上给予自拟豁痰开窍方联合针灸治疗,比较两组中医证候积分、神经功能缺损和运动功能评分[美国国立卫生院卒中量表(NIHSS)评分、Fugl-Meyer运动功能(FMA)评分]、认知功能和日常生活活动能力评分[蒙特利尔量表(MoCA)评分、日常生活活动量表(ADL)评分]、血清一氧化氮(NO)和血浆内皮素(ET-1)以及血管性假血友病因子(v WF)水平、不良反应。结果研究组治疗后中医证候积分、NIHSS、MoCA评分改善均优于对照组(P<0.05),两组治疗后FMA、ADL评分均明显上升,但差异无统计学意义(P>0.05);两组治疗后血清NO水平均明显上升,研究组上升幅度优于对照组(P<0.05),两组之治疗后血清ET-1、vWF水平明显下降,研究组下降幅度优于对照组(P<0.05);研究组不良反应发生率7.14%低于对照组28.57%(P<0.05)。结论脑梗死患者采用自拟豁痰开窍方联合针灸治疗临床疗效明显,可明显改善其神经功能缺损、运动功能、认知功能、日常生活活动能力以及血管内皮功能,且安全性较好,值得临床推广。Objective To explore the clinical effect of self-made Huotan Kaiqiao prescription combined with acupuncture in the treatment of cerebral infarction.Methods A total of 84 patients with cerebral infarction who were admitted to the hospital were enrolled.They were divided into study group and control group by random number table method,42 cases in each group.The control group was given routine treatment,while study group was given self-made Huotan Kaiqiao prescription combined with acupuncture on basis of control group.The scores for TCM syndrome,the National Institutes of Health Stroke Scale(NIHSS),Fugl-Meyer Assessment,Montreal Cognitive Assessment(MoCA)and Activity of Daily Living(ADL),levels of serum nitric oxide(NO),plasma endothelin(ET-1)and von Willebrand factor(vWF),and adverse reactions were compared between the two groups.Results After treatment,improvement of TCM syndrome,NIHSS,and MoCA scores in study group was better than that in control group(P<0.05),but no significant differences were found between the two groups in FMA and ADL scores(P>0.05).After treatment,level of serum NO was significantly increased in both groups,which was better in study group than control group(P<0.05),while levels of serum ET-1 and v WF were significantly decreased,which were better in study group than control group(P<0.05).The incidence of adverse reactions in study group was lower than that in control group(7.14%vs 28.57%)(P<0.05).Conclusion The clinical effect of self-made Huotan Kaiqiao prescription combined with acupuncture is significant in the treatment of cerebral infarction,which can significantly improve neurological deficits,motor function,cognitive function,activities of daily living,and vascular endothelial function,with good safety.
